MSCT对肾上皮样血管平滑肌脂肪瘤与肾透明细胞癌的鉴别诊断价值

Value of MSCT in differential diagnosis of renal epithelioid angiomyolipoma and renal clear cell carcinoma

目的探讨多排螺旋CT增强扫描对肾上皮样血管平滑肌脂肪瘤(EAML)与肾透明细胞癌(ccRCC)的鉴别诊断价值。方法回顾性分析经手术病理证实的12例肾上皮样血管平滑肌脂肪瘤及24例肾透明细胞癌的生长方式、楔形征、是否含脂肪、钙化、假包膜、平扫密度、强化特征及粗大瘤内血管等CT征象对两组病例的鉴别诊断价值。结果两组间如生长方式、楔形征、平扫密度、强化特征及粗大瘤内血管等征象比较,差异均有统计学意义(P <0. 05)。两组间如性别、边界、皮质掀起征、强化是否均匀及钙化灶等征象比较,差异均无统计学意义(P> 0. 05)。结论观察病灶的生长方式、平扫密度差异、强化特征、粗大瘤内血管等CT特征,对于鉴别EAML与ccRCC具有重要的诊断价值。

Objective To investigate the differential diagnosis between the renal epithelioid angiomyolipoma（EAML） and the renal cell carcinoma（ccRCC） by enhanced CT scan. Methods We retrospectively analyzed the growth pattern,the transparent wedge sign,fat,calcification,pseudocapsule,unenhanced attenuation,enhancement pattern,gross tumor vascular sign,between the 12 EAML and 24 ccRCC,which were confirmed by pathology. Results There were statistical differencs between the growth pattern,transparent wedge sign,unenhanced attenuation,enhancement pattern and gross tumor vascular sign（P〈0. 05）.While,the gender,boundary,cortex up sign,even or uneven enhancement and calcification had no statistical difference（P〉0. 05）. Conclusion The growth pattern,unenhanced attenuation,enhancement pattern,gross tumor vascular sign had diagnostic value in differentiating the EAML and ccRCC.
