噻托溴铵辅助支气管肺泡灌洗治疗支气管扩张合并肺部感染的临床效果

Clinical effect of Thiotropium Bromide assisted bronchoalveolar lavage in the treatment of bronchiectasis with pulmonary infection

目的探讨噻托溴铵辅助支气管肺泡灌洗(BAL)治疗支气管扩张合并肺部感染的临床效果。方法选取我院2016年1月~2017年8月收治的52例支气管扩张合并肺部感染患者,根据治疗方法的不同分成对照组和观察组,每组各26例。对照组采用BAL治疗,观察组采用噻托溴铵辅助BAL治疗,观察两组肺功能、血气指标变化及BODE指数评分、生活质量指数量表(QLI)评分。结果治疗后,观察组FVC、FEV1、FEV1%均高于对照组(P<0.05);观察组治疗后Pa CO_2低于对照组,Pa O_2/Fi O_2高于对照组(P<0.05);观察组治疗后BODE指数评分明显低于对照组,QLI评分显著高于对照组,差异均有统计学意义(P<0.05)。结论噻托溴铵辅助BAL用于支气管扩张合并肺部感染可有效改善患者肺功能、血气情况,提高生活质量,临床应用价值高。

Objective To investigate the clinical effect of Thiotropium Bromide assisted bronchoalveolar lavage（BAL）in the treatment of bronchiectasis with pulmonary infection. Methods A total of 52 cases of bronchiectasis with pulmonary infection patients in our hospital from January 2016 to August 2017 were selected as the objects, all the patients were divided into the control group and the observation group according to the difference treatment methods,26 cases in each group. The control group was treated with BAL, the observation group was treated with Tiotropium Bromide assisted BAL treatment. The lung function, blood gas index and BODE index score and quality of life index（QLI） score were observed between the two groups. Results After treatment, the level of FVC, FEV1, FEV1% in the observation group were higher than those of the control group（P〈0.05）. The level of Pa CO2 in the observation group after treatment was lower than that in the control group, the level of PaO2/FiO2 in the observation group after treatment were higher than those in the the control group（P〈0.05）. The BODE index score after treatment of the observation group was obviously lower than that of the control group, the QLI scores after treatment of the observation group was significantly higher than that of the control group, the differences were statistically significant（P〈0.05）. Conclusion Thiamethoxam bromide auxiliary BAL in the treatment of patients with pulmonary infection can effectively improve the pulmonary function, blood gas, improve the quality of life, with high clinical value.
