单环缝扎法治疗小儿尿道下裂术后3mm以下尿道皮肤瘘的临床研究

Simple suture ligation technique for repairing small urethrocutaneous fistulae( ≤3 mm) after hypospadias surgery

目的总结采用单环缝扎法治疗小儿尿道下裂术后3 mm以下尿道皮肤瘘的临床经验及疗效。方法选取2014年7月至2016年10月在本院收治的18例尿道下裂术后尿瘘的患儿作为研究对象。平均手术年龄为6岁(2岁5个月至11岁9个月)。平均尿瘘的直径2. 25 mm(1～3 mm)。所有患儿采用单环缝扎法修复尿道下裂术后尿道皮肤瘘。结果尿瘘均位于阴茎体部,其中阴茎体远端5例,阴茎体中部10例,阴茎体近端3例。术后对所有患儿进行门诊随访,平均随访时间20个月(9～36个月)。18例患儿术后恢复良好,均无尿瘘复发,无排尿困难。结论采用单环缝扎法修复尿道下裂术后较小UCF(≤3 mm)患儿的疗效确切,操作简单,值得临床推广应用。

Objective To describe a simple and effective technique for repairing small urethrocutaneousfistula （UCF） after hypospadias repair. Methods This procedure was performed in 18 patients with solitaryfistula after primary hypospadias repair. The mean age was 6 years （ range：2 years 5 months to 11 years 9months）. The average diameter of fistulae was 2. 25 （1 - 3） mm. There was no onset of perioperat ive complications. All UCFs were fixed by suture ligation. Results All fistulae were located on penile shaft,including anterior （n = 5）,middle （n = 10） and posterior （n = 3）. The average followup period was 20 （9 - 36） months. Norecurrent fistula was observed. None of them had voiding difficulties. Conclusion Simple suture ligation is effective for repairing small UCF （diameter≤3mm）.
