妊娠期合并子宫肌瘤对妊娠结局的影响

The Effect of Pregnancy Associated with Uterine Fibroids

目的研究妊娠期合并子宫肌瘤对妊娠结局的影响。方法选取妊娠期合并子宫肌瘤者62例作为观察组;随机选取同期分娩、正常孕妇62例作为对照组。比较2组妊娠期及分娩期并发症的发生几率,孕妇分娩方式以及新生儿情况。结果观察组肌瘤变性、先兆早产、先兆流产、胎位异常、前置胎盘、胎盘窘迫、宫缩乏力、胎盘早剥及产后出血的发生率分别为6.45%,14.52%,19.35%,12.90%,16.13%,9.68%,24.19%,8.06%,30.65%,均明显高于对照组。观察组的剖宫产率为61.29%,高于对照组的22.58%,差异有统计学意义。观察组新生儿窒息、低体重儿、低Apgar评分的发生率分别为6.45%,12.9%,9.68%,均高于对照组,差异有统计学意义。结论妊娠期合并子宫肌瘤患者易发生妊娠并发症,增加了剖宫产率,且新生儿结局不良,应引起临床注意。

Objective To investigate the effect of pregnancy associated with uterine fibroids. Methods 62 patients with uterine fibroids were selected as the observation group. 62 normal pregnant women were randomly selected as the control group. To compare the incidence of complications during pregnancy and childbirth,maternal delivery and neonatal condition. Results The incidence of myoma degeneration,aura,threatened abortion,premature abnormal position,placenta previa,placenta distress,contractions fatigue,placental abruption and postpartum hemorrhage in the observation group were 6. 45%,14. 52%,19. 35%,12. 90%,16. 13%,9. 68%,24. 19%,8. 06%,30. 65%,which were significantly higher than those of the control group,the difference was statistically significant. The rate of cesarean section in the observation group was 61. 29%,which was higher than22. 58% in the control group,and the difference was statistically significant. The incidence of neonatal asphyxia,low weight and low Apgar score in the observation group were 6. 45%,12. 90%,9. 68%,all higher than those of the control group,and the difference was statistically significant. Conclusion Complications in pregnancy associated with uterine fibroids are easy to occur,the cesarean section rate is increased,and the neonatal outcome is poor,which should arouse the clinical attention.
