麻醉时应用右美托咪定对胃癌根治术患者PaO_2、PaCO_2及氧合指数的影响分析

The Effect of Dexmedetomidine Amesthesia in Radical Operation for Gastric Cancer Patients on PaO_2、PaCO_2 and Oxygenation Index

目的研究分析麻醉时应用右美托咪定对胃癌根治术患者PaO_2、PaCO_2及氧合指数的影响。方法胃癌根治术患者80例作为研究对象。采用数字法随机分为观察组40例以及对照组40例,观察组患者使用右美托咪定,对照组患者使用咪达唑仑,对比观察两组患者PaO_2、PaCO_2及氧合指数变化。结果观察组的优良率为97.50%(39/40),显著高于对照组的70.00%(28/40)(P<0.05)。两组患者均能够将药物泵入的速度进行调整,使得Ramsay评分在理想范围内,观察组0~3分患者为90.00%(36/40),显著高于对照组的60.00%(24/40)。7~10分患者为0.00%(0/40),显著低于对照组的10.00%(4/40)(P<0.05)。观察组患者被唤醒时间为(0.17±0.02)min,显著低于对照组的(1.63±0.57)min(P<0.05)。两组经过不同方式麻醉处理之后,观察组的PaO_2上升趋势明显大于对照组,PaCO_2及氧合指数降低趋势大于对照组(P<0.05)。结论麻醉时应用右美托咪定对胃癌根治术患者PaO_2、PaCO_2及氧合指数可以进行较为明显的调节,有效缩短患者麻醉后恢复时间,效果较好,值得临床推荐使用。

Objective To study the effect of application of dexmedetomidine anesthesia in radical operation for gastric cancer patients on PaO2,PaCO2 and oxygenation index. Methods 80 cases of gastric cancer patients numbers were randomly divided into the observation group and 40 cases in the control group,each with 40 cases dexmedetomidine was used in the observation group,the control group was treated with midazolam,comparison of the 2 groups of patients PaO2,PaCO2 and oxygenation index change were made. Results The excellent rate of the observation group was 97. 50%（ 39/40）,which was significantly higher than the control group 70%（ 28/40）（ P〈0. 05）. The patients were able to adjust the speed of drug infusion,the Ramsay score in the observation group,90% patients with 0 - 3（ 36/40）,which was significantly higher than the control group 60%（ 24/40）,7 -10 patients was 0%（ 0/40）,significantly lower than the control group 10%（ 4/40）（ P〈0. 05）. Patients in the observation group wake up time was 0. 17 ± 0. 02 min,which was significantly lower than the control group of 1. 63 ± 0. 57 min（ P〈0. 05）. The 2 groups through different ways of anesthesia after the treatment,the observation group PaO2 increased more significantly than that of the control group,PaCO2 and oxygenation index decreased more significantly than the control group（ P〈0. 05）. Conclusion The effect of dexmedetomidine anestesia in radical operation for gastric cancer patients on PaO2,PaCO2 and oxygenation index can make obvious adjustment,shorten recovery time,the effect is good,it is worthy of recommendation.
