早期康复护理干预应用于腰椎退变性疾病腰椎椎间融合术患者中的效果观察

Effect of Early Rehabilitation Nursing Intervention on Lumbar Intervertebral Fusion in Lumbar Degenerative Diseases

目的探讨腰椎退变性疾病腰椎椎间融合术患者行早期康复护理干预的效果。方法选取武警浙江省总队医院嘉兴医院2016年12月—2017年12月106例行腰椎椎间融合术的腰椎退变性疾病患者,按照护理方法分为对照组和观察组,每组各53例。评价两组患者干预前、干预1个月和3个月时Oswestry功能障碍评分、Barthel指数评分、下腰疼痛程度和抑郁评分。结果两组患者干预1个月、3个月时Oswestry功能障碍评分较干预前明显下降,观察组干预1个月、3个月时Oswestry功能障碍评分明显低于对照组,P<0.05。两组干预1个月、3个月时Barthel指数评分明显高于干预前,观察组干预1个月、3个月时Barthel指数评分明显高于对照组,P<0.05。结论腰椎退变性疾病腰椎椎间融合术患者行早期康复护理干预有助于缓解其疼痛,改善其躯体功能,提高其日常生活能力,对缓解其抑郁症状具有重要意义。

OBJECTIVE To investigate the effect of early rehabilitation nursing intervention on patients with lumbar degenerative lumbar interbody fusion. METHODS 106 Patients with lumbar degenerative diseases undergoing lumbar interbody fusion in the hospital from December 2016 to December 2017 were grouped into a control group and an observation group according to nursing methods,with 53 cases in each group. The control group gave routine nursing care, and the observation group performed early rehabilitation nursing intervention on the basis of the control group. The Oswestry dysfunction score, Barthel index score, degree of lower back pain,and depression score were evaluated before and after the intervention for 1 month and 3 months. RESULTS Oswestry disability score was significantly decreased in the two groups at 1 month and 3 months after intervention. The Oswestry disability score was significantly lower in the observation group at 1 month and 3 months than in the control group（P〈0.05）. The Barthel index score was significantly higher in the two groups at 1 month and 3 months than before the intervention. The Barthel index score was significantly higher in the observation group at 1 month and 3 months than in the control group, P〈0.05. The VAS scores and SDS scores at the first and third month after intervention in the two groups were significantly lower than before the intervention. The VAS and SDS scores at the lower back were significantly lower in the observation group at 1 month and 3 months after intervention than in the control group（P〈0.05）. CONCLUSION Early rehabilitation nursing intervention for patients with degenerative lumbar degenerative lumbar inter-body fusion could help relieve pain, improve physical function, improve daily life ability and relieve the depressive symptoms.
