磁共振成像和磁共振血管造影对脑动静脉畸形的诊断评价

摘要 目的 探讨 MRI和 MRA ,尤其后者对脑动静脉畸形 (AVM)诊断价值和限度。 材料与方法 对 16例 AVM者行MRI和 MRA检查并与 DSA进行对照研究 ,分别观察供血动脉、血管巢和引流静脉情况。 结果 与 DSA对照 ,显示供血动脉 ,MRA与其无显著差异 (P>0 .0 5 ) ,MRI与其有极显著差异 (P<0 .0 0 1) ;血管巢大小 ,MRI、MRA与其均无显著差异 (P>0 .0 5 ) ;引流静脉 ,MRI与其有显著差异 (P<0 .0 0 1)。 结论  MRA与 MRI联合应用可准确显示供血动脉、血管巢和部分引流静脉 ,是一种评价 AVM血管内治疗、手术切除或立体定向放疗及效果观察的有效无创性检查方法。随着 MRA新技术的不断开发、完善 ,将部分取代有创性 Purpose To evaluate the value and limitation of MRI, especially MRA in the diagnosis of arteriovenous malformation(AVM) of brain. Materials and Methods The results of MRI and MRA in 16 cases with AVM were compared with that of DSA. The feeding arteries, nidus and draining veins were observed respectively. Results Correlated with DSA, MRA has no significantly difference in demonstrating the feeding arteries( P >0.05), but MRI has significantly difference ( P <0.001); Both MRA and MRI have no significantly difference in manifesting the size of AVM ( P >0.05); MRI has significantly difference in showing the draining veins ( P <0.001). Conclusions Combination of MRI and MRA can accurately demonstrate the feeding arteries, nidus and some draining veins. MRA is an effective no invasive method in evaluating the intravascular treatment, surgical resection, sterotatic radiotherapy and fellow up cases of AVM. With the new technical development, MRA will partially replace invasive DSA in the diagnosis of AVM.
