早期抗阻训练联合心身干预对髋关节置换术后患者功能恢复和不良情绪的影响

Effects of early resistance training combined with mental and physical intervention on functional recovery and negative emotions in patients after hip replacement

目的 探讨早期抗阻训练联合心身干预对髋关节置换术（THA）患者术后功能恢复及不良情绪的影响.方法 以河南省商丘市第一人民医院2016年1月—2017年6月择期行THA治疗的120例患者为研究对象,均行早期抗阻训练联合心身干预,持续到术后3个月,比较手术前后SDS评分、SAS评分、Harris髋关节功能评分及WHO生活质量量表（WHOQOL-100）评分情况,统计患者并发症发生率.结果 术后3个月,120例患者SAS评分为（22.19±3.97）分、SDS评分为（21.08±4.16）分,均比术前下降,差异有统计学意义（t值分别为34.930、27.898;P〈0.01）;术后3个月Harris评分为（83.32±5.79）分,WHOQOL-100量表总分为（20.16±3.92）分,均高于术前,差异有统计学意义（t值分别为-55.775、-76.853;P〈0.01）.干预期间发生深静脉血栓2例,肺部感染1例,并发症总发生率2.50％.结论 早期抗阻训练联合心身干预能改善THA患者术后不良情绪及心理状况,促进髋关节功能恢复并提高生活质量.

Objective To explore the effects of early resistance training combined with mental and physical intervention on functional recovery and negative emotions in patients after hip replacement. Methods From January 2016 to June 2017, a total of 120 patients with scheduled hip replacement were selected as subjects in the First People＇s Hospital of Shangqiu of He＇nan Province. Patients all accepted early resistance training combined with mental and physical intervention lasting three months after surgery. We compared the score of Self- rating Depressive Scale （SDS）, Self-rating Anxiety Scale （SAS）, Harris Hip Score, WHO Quality of Life （WHOQOL-100） before and after surgery and calculated the patients＇ incidence of complications. Results The score of SDS and SAS of 120 patients was （22.19 ＋ 3.97） and （21.08 -＋ 4.16） respectively three months after surgery lower than those before surgery with significant differences （t=34.930, 27.898; P 〈 0.01）. The score of Harris hip score and WHOQOL-100 was （83.32 ＋ 5.79） and （20.16 ＋ 3.92） respectively three months after surgery higher than those before surgery with significant differences （t=-55.775, -76.853; P 〈 0.01）. There were two cases with deep vein thrombosis and one case with pulmonary infection during intervention along with 2.50% for the total incidence of complications. Conclusions Early resistance training combined with mental and physical intervention can improve negative emotions as well as psychologic status, promote hip joint functional recovery and improve the quality of life of patients after hip replacement.
