慢性肾衰竭并发感染患者行血液吸附联合血液透析治疗的临床效果

Clinical effect of blood adsorption combined with hemodialysis treatment in patients with chronic renal failure complicated with infection

目的探究慢性肾衰竭并发感染患者行血液吸附联合血液透析治疗的临床效果。方法选择2014年8月-2016年8月于医院接受治疗的90例慢性肾衰竭并发感染患者作为研究对象,并随机分为A组、B组和C组,每组各30例,A组患者给予血液透析治疗,B组患者给予血液透析滤过治疗,C组患者给予血液吸附联合血液透析治疗,对三组患者治疗前后的生活质量评分、血清甲状旁腺素水平(iPTH)、感染症状缓解情况以及并发症发生情况进行综合评价。结果治疗后,C组患者的体能、日常生活受限、躯体疼痛与健康状况生活质量指标优于A组和B组患者(P<0.05);C组患者的血清iPTH水平为(110.34±32.12)ng/L,优于A组患者的(279.69±36.42)和B组患者的(265.26±35.36)(P<0.05);经过不同血液净化方式,C组患者感染缓解情况优于A组和B组患者(Z=10.274,P=0.001)。结论对慢性肾衰竭并发感染患者给予血液吸附联合血液透析治疗,能够提升患者的生活质量,改善患者的ipTH水平,且并发症少,值得临床推广应用。

OBJECTIVE To explore the clinical effect of blood adsorption combined with hemodialysis in patients with chronic renal failure complicated with infections.METHODS A total of 90 patients with chronic renal failure complicated with infections treated in our hospital from Aug.2014 to Aug.2016 were selected.They were randomly divided into three groups：group A,group B and group C,with 30 cases in each group.Patients in group A received hemodialysis treatment,in group were given hemodiafiltration therapy,and in group C were given blood adsorption combined with hemodialysis treatment.The quality of life score,serum parathyroid hormone levels（iPTH）,remission of infection and the occurrence of complications of the three groups before and after treatment were evaluated.RESULTS After treatment,the quality of life,limitation of daily life and quality of life index of somatic pain and health status of group C were significantly higher than those of group A and group B（P〈0.05）.The level of serum iPTH in group C was（110.34±32.12）ng/L,which was significantly better than（279.69±36.42）of group A and（265.26±35.36）of group B（P〈0.05）.After the different blood purification methods,the remission of group C was significantly better than that of group A and group B（Z =10.274,P =0.001）.CONCLUSIONBlood adsorption combined with hemodialysis can improve the quality of life and improve the level of ipTH in patients with less complications in the treatment of patients with chronic renal failure complicated with infection,which is worth clinical popularizing and application.
