不同程度阿尔茨海默病患者大脑异生皮质区CT血流灌注水平及其与精神行为异常的相关性

CT blood perfusion in the cerebral cortex of patients with different degrees of Alzheimer's Disease and its correlation with mental behavior disorder

目的探讨不同程度AD大脑异生皮质区CT血流灌注水平及与精神行为异常的相关性。方法选取我院AD患者89例,采用临床痴呆评定量表评分分轻度AD组(39例),中度AD组(32例),重度AD组(18例),选同期健康者30例为对照组,分析不同AD程度TTP和NPI评分相关性。结果不同程度AD患者不同感兴趣区TTP值较对照组有显著性差异(P<0.05),且中、重度AD组TTP显著较对照组延长,P<0.05。不同程度AD组NPI评分差异显著(P<0.05)。经相关性分析,轻、中、重度AD组和脑CT血流参数TTP与NPI评分均存在负相关性(P<0.05)。结论阿尔茨海默病患者大脑异生皮质区缺血性改变可能导致患者精神行为发生,且随病情进展损害加重。

Objective To investigate the CT blood perfusion level in the cerebral cortex of patients with different degrees of Alzheimer＇s disease（AD）and its correlation with mental behavior disorder. Methods 89 cases of AD patients in our hospital were selected,patients were divided into the mild AD group（39 cases）,the moderate AD group（32 cases）,and the severe AD group（18 cases）via the CDR scale. And 30 cases of healthy people were set as the control group. The correlation between TTP and NPI scores of different AD degree patients was analyzed. Results Blood perfusion parameters TTP in different interest regions among different AD degree patients and the control group had significant difference（P〈0.05）,and the TTP of the moderate and severe AD patients were significantly longer than that of the control group（P〈0.05）. There were significant differences in NPI scores among patients with different degrees of AD（P〈0.05）. CT cerebral blood flow parameters TTP of Mild,moderate and severe AD patients had a negative correlation with NPI scores in these patients（P〈0.05）. Conclusions Cerebral cortex ischemic changes of patients with Alzheimer＇s disease may be the causes of the damages in their mental behavior disorder,and mental behavior damage increases with the progression of the disease.
