同种异体腓骨移植锁定钢板固定治疗C2型肱骨近端骨折

Curative effect of allograft fibula transplantation locking plate fixation on type C2 proximal humeral fractures

目的 探讨同种异体腓骨移植锁定钢板固定治疗C2型肱骨近端骨折的疗效.方法 选择在医院住院采用同种异体腓骨移植锁定钢板固定治疗的C2型肱骨近端骨折患者32例,回顾分析其临床资料,根据影像学检查评估骨折愈合情况,通过测量颈干角和肱骨头高度、前臂、肩及手功能障碍、健康情况、总体治疗效果及并发症发生情况.结果 本研究32 例C2 型肱骨近端骨折患者中,平均手术时间为2.1 h;术中出血量为（287.4 ± 38.7）ml;切口长度为（13.4 ±0.7） cm;切口均为 I 期愈合;骨折愈合时间为（18.4 ±2.2）周;并发症发生率为9.38%;前臂、肩及手功能DASH量表评分为（13.1 ±6.8）分;患者健康情况SF-36量表评分为（86.7 ±11.4）分;根据Neer骨折治疗效果评分标准进行评价,优良率为90.62%.结论 同种异体腓骨移植锁定钢板固定治疗C2型肱骨近端骨折临床效果良好,不易发生内固定失败情况.

Objective To investigate the effect of allograft fibula transplantation locking plate fixation on type C2 proximal humeral fractures.Methods Thirty-two patients with C2 type proximal hu-meral fractures underwent allograft fibula transplantation locking plate fixation were selected.The clinical data were retrospectively analyzed , according to the imaging examination assessment of fracture healing , by measuring the neck stem Angle and humerus head height , arm, shoulder and hand function obstacle , health situation, the general therapeutic effects and complications were observed.Results Among the 32 type C2 patients with proximal humeral fractures , the average operation time was 2.1 h; the average intraoperative blood loss was （287.4 ±38.7） ml; the average length of incision was （13.4 ±0.7） cm;the incision was stage I heal ; the average fracture healing time was （18.4 ±2.2） weeks; the incidence of complications was 9.38%; DASH rating scale for forearm, shoulder and hand was 13.1 ±6.8; the pa-tients health status for SF-36 scale score was 86.7 ±11.4; According to Neer to evaluate the treatment effect criteria of fracture, the fine rate was 90.62%.Conclusions The clinical effect of allograft fibula transplantation locking plate fixation on proximal humeral fractures C 2 type is good, which is not easy to fail in internal fixation.
