肱三头肌内外侧入路与肱三头肌舌形瓣入路双钢板手术治疗肱骨髁间骨折的疗效分析

Analysis on the efficacy of bilateral approach of triceps and triceps tongue-shaped flap approach double steel plate internal fixation in the treatment of humeral intercondylar fracture

目的探讨肱三头肌内外侧入路与肱三头肌舌形瓣入路双钢板手术治疗肱骨髁间骨折的临床效果,进行比较分析。方法选取2012年1月—2017年12月我科收治50例肱骨髁间骨折患者作为研究对象,根据AO分型,C1型13例,C2型25例,C3型12例,分为肱三头肌内外侧入路组(26例)及肱三头肌舌形瓣入路组(24例)进行手术,比较两组手术时间、出血量、功能锻炼时间及肘关节Mayo评分。结果与肱三头肌舌形瓣入路组比较,肱三头肌内外侧入路组手术时间较短、出血量较少、功能锻炼时间早、Mayo评分较高,差异均有统计学意义(P<0.05)。结论对于肱骨髁间骨折双钢板手术治疗的入路,肱三头肌内外侧入路优于肱三头肌舌形瓣入路。

Objective To study the clinical efficacy of bilateral approach of triceps and triceps tongue- shaped flap approach double steel plate internal fixation in the treatment of humeral intercondylar fracture. Methods Fifty cases of patients suffered from humeral intercondylar fracture and hospitalized in our hospital during Jan, 2012 and Dec, 2017 were enrolled in this study. According to AO classification, there were 13 cases of C1, 25 cases of C2 and 12 cases of C3.Twenty-six cases were treated trough bilateral approach of triceps, while the others 24 cases were treated by triceps tongue-shaped flap approach. The time of operation, bleeding volume, time of functional exercise and Mayo scores of the two groups were compared.Results Cases in bilateral approach of triceps group with less operation time, less bleeding volume, earlier functional exercise and higher Mayo scores than the triceps tongue-shaped flap approach group, the differences were statistical significant （ P〈0. 05）. Conclusions In surgery treatment with double steel plate internal fixation for humeral intercondylar fractures, bilateral approach of triceps was more beneficial than triceps tongue-shaped flap approach.
