中西医内外联合治疗中重度膝骨关节炎临床观察

Clinical observation of combined treatment of Chinese and Western medicine and traditional Chinese and Western Medicine on moderate and severe knee osteoarthritis

目的：观察口服通筋正骨胶囊与局部注射玻璃酸钠联合治疗中重度膝骨关节炎的临床疗效。方法：将近2年收治的70例中重度膝骨关节炎患者随机分为对照组和观察组，每组35例。对照组给予关节腔内注射玻璃酸钠；观察组口服通筋正骨胶囊，关节腔内注射玻璃酸钠；5周为1个疗程，观察2组患者临床疗效。治疗前后采用膝关节视觉模拟评分法（VAS）、西安大略和麦马斯特大学骨关节炎指数可视化量表（WOMAC）及总体疗效进行评定。结果：本法治疗后膝骨关节炎患者VAS、WOMAC评分较治疗前显著降低，观察组优于对照组，具有统计学意义（P〈0．05）；观察组患者总有效率为82．86％，明显优于对照组的54．29％，差异具有统计学意义（P〈0．01）。结论：通筋正骨胶囊与玻璃酸钠内外联合治疗中重度膝骨关节炎，在改善患者临床症状、减轻关节疼痛、改善关节功能方面疗效确切，值得临床推广应用。

Objective： To observe the clinical curative effect on the Treatment of Medium - severe Knee Osteoarthritis by External and Internal Application of Tongjin - bonesetting Capsules Combined with Sodium Hyaluronate. Methods： 70 patients in recent two years with medium - severe knee osteoarthritis were randomly divided into control group and treatment group, 35 cases in each group. The control group of intra - articular injection of sodium hyaluronate of giving; In control group, sodium hyaluronate was injected into the patient＇s joint cavity; In treatment group, with the injection of sodium hyaluronate, patients were given oral tongjin-bonesetting capsules. Taking 5 weeks as a period of treatment, observe the clinical efficacy of two groups of patients. Before and after the treatment, the two methods were used to evaluate the overall curative effect, which was VAS （ Knee joint visual analogue scale） and WOMAC （Western ontario and McMaster university osteoarthritis index ）. Results： After the treatment, the scores of VAS, WOMAC were significantly lower than that before the treatment, and the result of the treatment group was better than the control group. And there were statistical significance （ P 〈 0.05） ; The total effective rate was 82.86% in the treatment group, it was significantly higher than the 54.29% in the control group. There were statistically significant （P 〈 0.01 ） in the difference. Conclusion： There was effective in improving clinical symptoms, reducing joint pain and improving joint function in the combined treatment. It was worthy of clinical application.
