品管圈改善2型糖尿病老年病人胰岛素注射依从性和血糖控制情况的观察

Improvement of insulin injection compliance and blood glucose control in elderly patients with type 2 diabetes mellitus by quality control circle

[目的]探讨品管圈在改善2型糖尿病老年病人胰岛素治疗的依从性及血糖控制情况中的应用效果,为糖尿病老年病人的临床治疗护理提供依据。[方法]选取本院90例2型糖尿病病人为研究对象,按随机数字表法分为常规组和观察组各45例。常规组给予糖尿病常规治疗与护理,观察组在常规组基础上实施品管圈护理管理。比较两组病人胰岛素注射治疗的依从性以及空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)水平控制情况。[结果]观察组病人胰岛素注射依从性明显高于常规组,差异有统计学意义(P<0.05);干预前两组病人FPG、2hPG和HbA1c水平比较差异无统计学意义(P>0.05),干预后观察组病人FPG、2hPG和HbA1c等指标水平均明显优于常规组,差异有统计学意义(P<0.05)。[结论]对老年2型糖尿病病人实施品管圈护理管理可有效提高病人的胰岛素注射依从性,明显改善病人的血糖控制情况。

Objective： To explore the effects of quality control circle（QCC） on the improvement of insulin injection compliance and blood glucose control in elderly patients with type 2 diabetes mellitus（T2DM）. Methods： Ninety elderly patients with T2DM were randomly divided into two groups：control group（ n =45） treated with routine treatment and nursing care,and treatment group（ n =45） treated with QCC in the basis of routine treatment and nursing care.The insulin injection compliance,fasting blood glucose（FPG）,postprandial 2 h blood glucose（2 hPG）,and glycosylated hemoglobin（HbA 1c） levels were observed. Results： The compliance of insulin injectionin in the observation group was significantly better than that in the control group（ P〈0.05）.There was no significant difference in the levels of FPG,2 hPG and HbA1c between the two groups before intervention（ P〉0.05）.The levels of FPG,2 hPG and HbA1c in the observation group were significantly better than those in the control group after intervention （ P 〈0.05）. Conclusions： QCC can effectively improve the compliance of insulin injection and the blood glucose control in elderly patients with T2DM.
