改良偏头法预防PICC置管误入颈内静脉的效果观察

Application of the improved method of turning head to prevent PICC tip into the internal jugular vein

目的探讨改良偏头法在预防外周静脉穿刺置入中心静脉导管(PICC)置管过程中导管误入颈内静脉的作用。方法按入组时间顺序将实施PICC置管的379例患者分为对照组(n=190)和观察组(n=189)。对照组在置管过程中采用常规转头方法来阻止导管误入颈内静脉,观察组采用改良的偏头法来阻止导管误入颈内静脉。结果观察组PICC导管误入颈内静脉的发生率4.76%(9/189),低于对照组的10.53%(20/190),差异有统计学意义(P<0.05)。结论在PICC置管过程中采用改良偏头法,能有效预防PICC导管误入颈内静脉,提高一次送管成功率。

Objective To evaluate the improved method of turning head in preventing peripherally inserted central catheter（ PICC） tip into the internal jugular vein. Methods Totally 379 patients undergoing PICC catheterization w ere assigned to the control group（ n = 190） and the observation group（ n = 189）. The routine turning head method w as used to prevent misplaced catheter tip into the internal jugular vein,and the improved method of turning head w as applied in observation group. Results The incidence of misplaced catheter into jugular vein w as low er in observation group than that of controls（ 4. 76% vs. 10. 53%,P 〈0. 05）. Conclusion The improved method of turning head may prevent the misplaced PICC tip into the internal jugular vein,w hich in turn improves the one-time success rate of puncture.
