梅花针刺络拔罐联合苗药复方治疗带状疱疹后遗神经痛的随机对照试验

Clinical observation of blood-letting puncture with plum blossom needle and cupping therapy combined with Miao medicine application in treating Post Herpetic Neuralgia

目的评价梅花针刺络拔罐联合苗药复方治疗PHN的临床疗效及安全性。方法将120例PHN患者按随机数字表法分为对照组和治疗组,每组60例。对照组患者给予梅花针刺络拔罐法治疗,治疗组患者在对照组处理基础上联合苗药复方治疗,两组患者均治疗4周。观察两组患者临床疗效、SF-MPQ-2量表评分和生活质量,并评价不良反应。结果VAS、感觉PRI、情感PRI评分、PPI和生活质量结果表明:时间因素之间均有统计学意义(P<0.05),说明各项指标随时间变化;但时间和两组之间的交互作用,没有统计学意义(P>0.05),说明时间因素的作用与分组无关;与治疗前相比,VAS评分治疗第1周无统计学意义,治疗第2、3、4周有统计学意义(P<0.05),其余指标在治疗各周均有统计学意义(P<0.05);与对照组相比较,治疗组患者在第1和2周,无统计学意义,第3和4周均有统计学意义(P<0.05)。出现轻微不良反应的例数仅3例,说明本研究方法安全可靠。结论梅花针刺络拔罐联合苗药复方外敷的综合治疗有效减轻PHN患者疼痛程度,疗效确切,安全可靠,值得借鉴使用。

Objective To investigate the clinical efficacy and safety of the blood letting puncture and cupping combined with Miao medicine external application in treating postherpetic neuralgia. Methods 120 PHN patients were randomly divided into control group and treatment group,each containing 60 cases. The patients in the control group were treated with plum blossom acupuncture and cupping therapy. The treatment group was treated with plum blossom acupuncture and cupping therapy combined with Miao medicine. Both groups were treated for 4 weeks. During the course of treatment,the clinical efficacy of the two groups was observed,the SF-MPQ-2 scale was rated,and the quality of life and the adverse reactions were evaluated. Results The results of VAS,sensory PRI,emotional PRI scores,PPI,and quality of life indicated that the indicators changed over time,but the effect of the time factor was independent to grouping. Compared with before treatment,there was no statistical significant difference in the first week based on VAS score,and the scores were obviously different in 2,3,and 4 weeks（P〈0. 05）. The other indicators were statistically different at each week（P〈0. 05）. Compared with the control group,the treatment effects were not significantly difference at the 1 st and 2 nd weeks,and there were statistical differences at the 3 rd and 4 th weeks（P〈0. 05）. Only 3 cases of mild adverse reactions were found,which indicated that the method was safe and reliable. Conclusion Plum blossom acupuncture and cupping combined with Miao medicine topical treatment of PHN could reduce the degree of pain,and be safe and reliable.
