摘要
目的探讨增强MRI直方图分析鉴别侧脑室中枢神经细胞瘤和室管膜瘤的应用价值。方法回顾性分析新乡市第一人民医院经手术病理证实的侧脑室中枢神经细胞瘤23例,室管膜瘤20例。收集该两种肿瘤的T1、T2加权和增强T1磁共振图像,用软件MaZda提取肿瘤的感兴趣区(region of interest,ROI),针对肿瘤最大层面的感兴趣区进行直方图分析,利用平均值、方差、偏斜度、峰度及第1、10、50、90、99百分位数特征,找出两种肿瘤之间的显著性差异。结果利用直方图提取出的9个参数特征中,平均值、方差及第50、90、99百分位数差异具有统计学意义(P<0.05),偏度、峰度及第1、10百分位数差异无统计学意义(P>0.05),并绘制相应ROC曲线,其中第99百分数鉴别性最好,ROC曲线下的最大面积为0.72,最佳阈值取145.5时,敏感性为65%,特异性75%。结论直方图分析可以提供更多量化信息特征,可用于鉴别侧脑室中枢神经细胞瘤和室管膜瘤。
Objective To investigate the diagnostic value of the histogram analysis derived from enhancement MR imaging in differentiating lateral ventricle central neurocytoma from ependymoma. Methods We retrospectively analyzed pathologically confirmed 23 cases of central neurocytoma and 20 cases of ependymoma. The T1 ,T2 and enhanced T1 weighted magnetic resonance images were collected,the tumor region of interest was extracted using the software of MaZda (region of interest,ROD ,histogram analysis for the maximum level of tumor region of interest, using the mean value ,variance,skewness,kurtosis,the 1,10,50,90,99 percentile characteristics was applied to find out the significant differences between the two kinds of tumors. Results In the 9 pa rameters which were extracted from histogram, the mean value,variance, 50th, 90th, 99th percentiles had the statistical significance (P〈0.05). The skewness,kurtosis,lth,10th percentiles showed no statistical significance(P〉0.05). ROC curve analysis of the 99 percentiles yielded the best area under the ROC curve (AUC:0. 72) ,sensitivity of 65% ,and specificity of 75%,with a cutoff value of 145.50. Conclusion Histogram analysis can provide more quantitative information characteristics, might be helpful for discriminating lateral ventricle central neurocytoma from ependymoma.
