带锁髓内钉治疗四肢创伤骨折后骨不连的临床效果

Clinical effect of interlocking intramedullary nail in the treatment of nonunion after traumatic fractures of extremities

目的探讨带锁髓内钉治疗四肢创伤骨折后骨不连的临床效果。方法选取我院收治的股骨创伤骨折后骨不连患者86例作为研究对象,根据其入院序号奇偶数分为对照组(43例,动力加压钢板固定治疗)与观察组(43例,带锁髓内钉治疗)。对比两组患者骨不连愈合情况、手术情况和并发症发生情况。结果观察组患者骨折愈合优良率明显高于对照组(P<0.05)。相较于对照组,观察组患者的手术时间更短,术中出血量和术后引流量更少(P<0.05)。观察组患者治疗后2个月愈合不良、关节僵硬、切口感染发生率均低于对照组(P<0.05)。结论带锁髓内钉治疗四肢创伤骨折后骨不连效果显著,可以减少手术时间及出血量,促进患者关节功能恢复,且术后并发症发生率较低,值得推广。

Objective To study the clinical effect of interlocking intramedullary nail in the treatment of nonunion aftertraumatic fractures of extremities. Methods Eighty-six patients with nonunion after femoral trauma fractures enrolled inour hospital were divided into control group （43 cases, treated with dynamic pressurized steel plate fixation） andobservation group （43 cases, treated with locking Internal nail treatment） according to the odd and even of admissionnumber. The healing of nonunion, operation status and the occurrence of complications were compared between the twogroups. Results The excellent and good rate of fracture healing in the observation group was significantly higher than thatin the control group （P〈0.05）. Compared with the control group, the operation time of the observation group was shorter,the intraoperative blood loss and postoperative drainage volume were less（P〈0.05）. The incidences of poor healing, jointstiffness and incision infection in the observation group were lower than those in the control group at 2 months aftertreatment （P〈0.05）. Conclusion Interlocking intramedullary nail in the treatment of nonunion after traumatic fractures ofextremities has significant effect, which can reduce the operation time and the amount of bleeding, promote the recoveryof joint function, and the incidence of postoperative complications is low, it is worth promoting.
