结核感染T细胞γ干扰素释放试验(TB-IGRA)在结核病诊断中的应用 被引量:11

摘要 目的探讨结核感染T细胞γ干扰素释放试验(TB-IGRA)诊断结核病的应用价值。方法对结核病患者86例、肺部其他疾病患者98例分别进行TB-IGRA试验、血清结核杆菌抗体及痰涂片抗酸染色检查,并对检测结果进行回顾性分析。结果结核病组、肺结核组及肺外结核组的TB-IGRA阳性率显著高于肺部其他疾病组,IFN-γ浓度高于肺部其他疾病组,差异有统计学意义(P<0.05)。涂阳肺结核和涂阴肺结核患者的TB-IGRA阳性率及IFN-γ浓度无统计学差异(P>0.05)。TB-IGRA试验诊断结核病的灵敏度显著高于结核抗体和涂片抗酸染色,特异度高于结核抗体(P<0.05)。结论 TB-IGRA试验对结核病的诊断有较高的灵敏度和特异度,对宝鸡地区结核病的临床诊断价值较大。 Objective To investigate the application value of tuberculosis interferon gamma release assay (TB-IGRA)in the diagnosis of tuberculosis. Methods A total of 86 cases of tuberculosis patients and 98 cases of other lung diseaseswere tested with TB-IGRA test, serum tubercle bacillus antibody and sputum smear acid-fast staining test, and the testresults were retrospectively analyzed. Results The TB -IGRA positive rates in the tuberculosis group, the pulmonarytuberculosis group and the extrapulmonary tuberculosis group were significantly higher than that in the other lungdiseases groups, and the IFN-γ concentrations were higher than that of the other lung disease groups (P〈0.05). Therewere no significant differences in TB-IGRA positive rates and IFN-γ concentrations between smear-positive and smear-negative pulmonary tuberculosis patients (P〉0.05). The sensitivity of the TB-IGRA test for diagnosis of tuberculosis washigher than that of the tuberculosis antibody test and smear acid-fast staining test, the specificity was higher than that ofthe tuberculosis antibody test (P〈0.05). Conclusion The TB-IGRA test has a high sensitivity and specificity for thediagnosis of tuberculosis, which has a higher clinical diagnostic value for tuberculosis in Baoji area.
