妊娠期肝内胆汁淤积症的临床特征

Clinical features of intrahepatic cholestasis in pregnancy

目的分析妊娠期肝内胆汁淤积症患者的临床情况。方法对本院收治的50例妊娠期肝内胆汁淤积症患者进行回顾性分析,参考肝功能指标情况分为试验A、B组,各25例。试验A组为轻度妊娠期肝内胆汁淤积症患者,试验B组为重度妊娠期肝内胆汁淤积症患者,另外将本院同期就诊的25例健康产妇作为参照组,统计和研究三组的围生儿结局和妊娠结局。结果试验A组及试验B组早产儿、低出生体重儿、胎儿窘迫、羊水1~3度污染、胎膜早破、剖宫产、产后出血占比均显著高于参照组,且试验B组高于试验A组(P<0.05)。结论重度妊娠期肝内胆汁淤积症患者的围生儿结局情况、妊娠结局情况和轻度妊娠期肝内胆汁淤积症患者相比更差,需要进行针对性治疗,以有效改善新生儿预后及妊娠结局。

Objective To analyze the clinical situation of intrahepatic cholestasis of pregnancy. Methods Fifty patientswith intrahepatic cholestasis of pregnancy in our hospital were selected and analyzed retrospectively. According to the liverfunction indexes, the patients were divided into experimental group A and experimental group B, with 25 cases in each group.The experimental group A was patients with mild intrahepatic cholestasis of pregnancy, and the experimental groupB waspatients with severe intrahepatic cholestasis of pregnancy. In addition, 25 cases healthy women admitted in our hospital at thesame time were set as reference group. The perinatal and pregnancy outcomes of the three groups were statistically analyzed.Results The proportions of premature births, low birth weight children, fetal distress, 1 -3 degrees of amniotic fluidcontamination, premature rupture of membranes, cesarean section and postpartum hemorrhage in the experimental groupAandB were significantly higher than those in the reference group, and those of the experimental groupB were significantlyhigher than the experimental groupA（P〈0.05） . Conclusion Among the patients with severe intrahepatic cholestasis duringpregnancy, the perinatal and pregnancy outcomes of children are worse than those with mild intrahepatic cholestasis duringpregnancy. Targeted treatment is needed to effectively improve the prognosis and pregnancy outcomes of newborns.
