预警性护理体系的构建及在AECOPD伴Ⅱ型呼吸衰竭患者中的干预效果

Construction of early warning nursing system and intervention effect in patients with AECOPD and type Ⅱ respiratory failure

目的分析警性护理体系的构建及在慢性阻塞性肺疾病急性加重(AECOPD)伴Ⅱ型呼吸衰竭患者中的干预效果。方法回顾性分析110例AECOPD伴Ⅱ型呼吸衰竭患者的临床资料,根据护理方式的不同将患者分为常规组(50例,常规护理)与观察组(60例,常规护理+预警性护理)。比较两组患者的抢救成功率、抢救成功患者护理前、后血气指标及再次住院率。结果两组患者的抢救成功率比较,差异不显著(P=0.504)。护理后,两组患者PaO2、pH均升高,PaCO2均降低,且观察组明显优于常规组(P<0.05)。观察组患者的再次住院率低于对照组(P=0.049)。结论在AECOPD伴Ⅱ型呼吸衰竭患者中,采用预警性护理可促进患者血气指标的改善、再次住院率的降低。

Objective To analyze the construction of early warning nursing system and intervention effect in patientswith acute exacerbation of chronic obstructive pulmonary disease （AECOPD） with type II respiratory failure. Methods Theclinical data of 110 patients with AECOPD and type II respiratory failure were retrospectively analyzed. According to thedifferent nursing methods, patients were divided into routine group （50 cases, treated with routine nursing） andobservation group （60 cases, treated with routine nursing＋early warning nursing）. The rescue success rates, the blood gasparameters before and after nursing and rates of re -hospitalization of the rescue successful patients between the twogroups were compared. Results There were no significant differences in the rescue success rates between the two groups（P =0.504）. After nursing, the PaO2 and pH increased and PaCO2 decreased in the two groups, and those in theobservation group were significantly better than the routine group （P 〈0.05） The rate of re -hospitalization in theobservation group was significantly lower than that of the routine group （P=0.049）. Conclusion Early warning nursingsystem in patients with AECOPD and type II respiratory failure can promote the improvement of blood gas indexes anddecrease the re-hospitalization rates.
