胺碘酮联合依那普利治疗高血压伴PAF的疗效及对心功能、房颤复发的影响

Curative effect of amiod arone combined with enalapril on hypertension complicated with PAF and its influence on cardiac function and recurrence of atrial fibrillation

目的:研究胺碘酮联合依那普利治疗高血压伴阵发性房颤(PAF)的疗效及对心功能、房颤复发的影响。方法:将我院收治的126例高血压伴阵发性房颤患者随机分为实验组(实施胺碘酮+依那普利治疗,n=63)与对照组(实施依那普利治疗,n=63),观察两组治疗前后心电图参数、心功能指标、房颤复发情况。结果:治疗后,两组Pmax、Pd、心率水平、Pd≥40ms比例均较治疗前显著下降,且实验组各指标下降幅度均大于对照组(P<0.05);治疗随访6个月后,两组心功能指标LADL、VEDD水平均较治疗前下降,LVEF水平较治疗前上升,且实验组各指标下降或上升幅度均大于对照组(P<0.05);实验组、对照组6个月中房颤复发次数分别为[(6.35±1.36)次和(8.36±2.01)次,P<0.05],发作时间分别为[(146.36±21.63)min和(169.63±32.51)min,P<0.05]。结论:胺碘酮联合依那普利能有效治疗高血压伴PAF,提高心功能,减少房颤复发次数与发作时间,值得临床推广。

Objective： To study the curative effect of amiodarone combined with enalapril on hypertension complicated with paroxysmal atr/al fibrillation （PAF） and its influence on cardiac function and recurrence of atrial fibrillation. Methods： 126 hypertensiαn patients with paroxysmal atrial fibrillation treated in our hospital were randomly divided into experimental group（n=63 ） and control group（n=63 ）. Both groups were given enalapril, besides, the experimental group was given additional amiodarone. The echocardiographic parameters, cardiac function indexes and recurrence rates of atrial fibrillation in the two groups were observed before and after treatment. Results： After treatment, the Pmax, Pd, heart rate and ratio of Pd≥40ms in the two groups were significantly decreased, and the change ranges in the experimental group were greater than those in the control group （P〈0.05）; after 6 months follow-up, the levels of cardiac function indexes LADL and VEDD in the two groups were obviously decreased, the LVEF levels in the two groups were significantly increased, and the change ranges in the experimental group were greater than those in the control group （P〈O.05）; the recurrence frequency and time of atrial fibrillation in the experimental group were respectively （6.35±1.36）and （146.36±21.63）min which were significantly lower than those in the control group （8.36±2.01）and （169.63±32.51）min （P〈0.05）. Conclusion： Amiodarone combined with enalapril which is worthy of clinical promotion, can effectively treat hypertension with PAF, improve heart function, and reduce the recurrence frequency and attack time of atrial fibrillation.
