膝骨性关节炎行关节镜术后联合中医辨证治疗对膝关节功能恢复的影响

Effect of Arthroscopic Surgery Combined with Traditional Chinese Medicine Syndrome Differentiation on Knee Joint Function Recovery of Knee Osteoarthritis

目的:探讨膝骨性关节炎行关节镜术后联合中医辨证治疗对膝关节功能恢复的临床疗效。方法:选择符合膝骨性关节炎诊断标准且行关节镜手术的240例患者,随机分成对照组120例,不采取任何治疗,仅进行功能锻炼;治疗组120例,根据舌脉症及局部表现进行中医辨证分型论治,结合功能锻炼。于术前、术后2周进行膝关节疼痛、肿胀、HSS评分,并进行临床疗效评定。结果:治疗组优良率89.17%,对照组为65.83%,两组比较差异有统计学意义(P<0.05)。术后2周,两组膝关节疼痛、肿胀、HSS评分分别与本组术前比较,差异均有统计学意义(P<0.05),治疗组膝关节疼痛、肿胀、HSS评分改善优于对照组(P<0.05)。治疗组瘀血阻滞型、阳虚寒凝型和肾虚髓亏型优良率分别为89.28%、81.26%、93.33%;对照组为66.67%、44.73%和79.31%。两组同证型优良率比较差异有统计学意义(P<0.05)。结论:关节镜术联合中医辨证治疗膝骨性关节炎有确切疗效,较单纯关节镜术治疗效果明显。

Objective： To observe the clinical efficct of knee arthroscopy combined with traditional chinese mediciue （TCM）syndrome differentiation on knee joint function recovery of knee osteoarthritis.Methods： 240 patients with knee osteoarthritis and arthroscopic surgery were randomly divided into control group of 120 cases, without any treatment, only functional exercise. 120 cases in the treatment group,TCM treatment based on syndrome differentiation according to the patient＇s tongue vein disease and local performance, combined with functional exercise.The knee joint pain, swelling and HSS score were evaluated before and 2 weeks after operation, and the clinical efficacy was evaluated.Results： The excellent and good rate of the treatment group was 89.17%, and that of the control group was 65.83%, The difference between the two groups was statistically significant （P〈0.05）. After the operation 2 weeks, The two groups of knee joint pain and swelling, and the HSS score were compared, the differences were statistically significant （P〈0.05）, the treatment group of knee joint pain and swelling, HSS score better than the control group （P〈0.05）.The excellent and good rates of blood stasis, Yang deficiency and kidney deficiency were 89.28%, 81.26% and 93.33% in the treatment group,and 66.67%, 44.73% and 79.31% in the control group ,respectively. There was significant difference between the two groups in the excellent and good rate of the same syndrome （P〈0.05）.Conclusion： Arthroscopic surgery combined with TCM syndrome differentiation has definite curative effect on knee osteoarthritis, and it is more effective than arthroscopy alone.
