乌鸡白凤丸联合舍曲林治疗产后抑郁症临床疗效观察

Clinical Research of Wuji Baifeng Pills combined with Sertraline in the Treatment of Postpartum Depression

目的:观察乌鸡白凤丸联合舍曲林治疗产后抑郁症的临床疗效及安全性。方法:将114例产后抑郁患者采用随机双盲对照法分为对照组和治疗组各57例,对照组患者给予舍曲林治疗,治疗组患者给予舍曲林和乌鸡白凤丸治疗,治疗8周。于治疗前和治疗2、4、8周后观察并比较两组患者的爱丁堡产后抑郁量表(EPDS)、临床疗效总评量表(CGI)、艾森贝格抗抑郁剂不良反应量表(SERS)和生活质量指数量表(QL-I),评价两组患者总体疗效及安全性。测定并比较两组患者治疗前后血清中5-羟色胺(5-TH)、雌二醇(E2)、黄体酮(P)水平。结果:治疗8周后,治疗组患者总有效率为92.98%,优于对照组的80.70%,差异具有统计学意义(P<0.05)。治疗前两组患者EPDS、CGI、QL-I评分比较差异无统计学意义(P>0.05);治疗8周后,两组患者的EPDS和CGI评分均明显低于治疗前,差异具有统计学意义(P<0.05);而QL-I评分明显高于治疗前,差异具有统计学意义(P<0.05)。治疗前两组患者5-HT、E2及P水平比较差异无统计学意义(P>0.05);治疗8周后,两组患者血清中5-HT、E2及P水平明显增高,差异具有统计学意义(P<0.05)。治疗前后两组患者的血、尿、便常规,肝肾功能、心电图和脑电图无明显异常。两组均未出现严重不良反应,其中对照组不良反应总例数10例,治疗组不良反应总例数4例,两组患者的不良反应总数和SERS评分比较差异有统计学意义(P<0.05)。结论:乌鸡白凤丸联合舍曲林治疗产后抑郁症疗效确切,可提高患者的生活质量且不良反应发生率低,并能有效调整抑郁症患者5-TH、E2和P水平,从而改善患者临床症状,值得临床推广应用。

Objective：To observe the clinical efficacy and safety of wuji baifeng pill combined with sertraline in postpartum depression depression.Methods：114 patients of postpartum depression were randomLy divided into treatment and control groups with 57 cases in each group.Sertraline was taken oral in control group for 8 weeks.Observation group took wuji baifeng pills combined with sertraline for 8 weeks.The Clinical efficacy and safety were assessed with the edinburgh postnatal depression scale（EPDS）,clinical global impression（CGI）scale,rating scale for side effects（SERS）and quality of life index（QL-I）before and at the end of the 2,4,8 weeks of the treatment.the serum levels of 5-HT,E2,and P were detected before and after the 8 weeks of the treatment.Results：After 8 weeks of treatment,the total effective rate was 92.98%in the treatment group,which was better than 80.70%in the control group.The difference was statistically significant（P〈0.05）.There was no significant difference in EPDS,CGI,QL-I scores between the two groups before treatment（P〈0.05）;after 8 weeks of treatment,EPDS and CGI scores of the two groups were significantly lower than before treatment,the difference was statistically significant（P〈0.05）;and QL-I scores were significantly higher than before treatment,the difference was statistically significant（P〈0.05）.There was no significant difference in the levels of 5-HT,E2 and P between the two groups before treatment（P〉0.05）;after 8 weeks of treatment,serum levels of 5-HT,E2 and P in the two groups were significantly increased（P〈0.05）.Before and after treatment,the routine blood,urine and stool,liver and kidney function,electrocardiogram and electroencephalogram of the two groups were normal.There were no serious adverse reactions in the two groups.There were 10 cases of adverse reactions in the control group and 4 cases in the treatment group.There were significant differences in the total number of adverse reactions and SERS score between the two groups（P〈0.05）.Conclusion：Wuji Baifeng pill combined with sertraline is effective in the treatment of postpartum depression,which can improve the quality of life of patients with low incidence of adverse reactions,and can effectively adjust the level of 5-TH,E2 and P in patients with depression,thereby improving the clinical symptoms of patients,worthy of clinical application.
