罗库溴铵用于老年患者腹腔镜胆囊切除术麻醉的效果观察

Anesthetic Effects of Rocuronium Bromide on Laparoscopic Cholecystectomy in Senile Patients

目的:探讨罗库溴铵用于老年患者腹腔镜胆囊切除术麻醉的效果及对血流动力学指标、肌肉松弛起效及持续时间的影响。方法:选取2016年1月至2017年12月解放军第一五九医院收治的拟行腹腔镜胆囊切除术的老年患者500例,以随机数字表法分为观察组和对照组,每组250例。对照组患者给予阿曲库铵,观察组患者给予罗库溴铵。比较两组患者的血流动力学指标[心率(HR)、平均动脉压(MAP)]、肌肉松弛起效及持续时间的差异。结果:麻醉诱导前,两组患者HR、MAP水平的差异无统计学意义(P>0.05);给药后1、3和10 min,观察组患者HR水平明显低于对照组,MAP水平明显高于对照组,差异均有统计学意义(P<0.05)。观察组患者平均肌肉松弛起效时间为(126.35±39.68)s,明显短于对照组的(194.53±41.27)s;平均肌肉松弛维持时间为(49.25±6.99)min,明显长于对照组的(32.86±6.34)min,差异均有统计学意义(P<0.05)。结论:罗库溴铵用于老年患者腹腔镜胆囊切除术麻醉,可有效稳定患者血流动力学指标,且起效较快,维持时间较长。

OBJECTIVE： To probe into the effects of rocuronium bromide on laparoscopic cholecystectomy in senile patients and the impact on hemodynamic indicators,muscle relaxation onset and duration. METHODS： 500 senile patients undergoing laparoscopic cholecystectomy admitted into the 159 th Hospital of PLA from Jan. 2016 to Dec. 2017 were extracted to be divided into observation group and control group via the random number table,with 250 cases in each. The control group was given atracurium,while the observation group received rocuronium bromide. Difference of hemodynamic indicators[HR and MAP],muscle relaxation onset and duration of two groups were compared. RESULTS：Before anesthesia induction,there was no statistically significant difference between two groups in HR and MAP（ P〉0. 05）. After administration of 1,3 and 10 min,HR of observation group was significantly lower than that of control group,and MAP was significantly higher than that of control group,with statistically significant differences（ P〈0. 05）.The average muscle relaxation onset time of observation group was（ 126. 35 ± 39. 68） s,significantly shorter than that of control group [（ 194. 53 ± 41. 27） s],and the average maintenance time of observation group was（ 49. 25 ± 6. 99） min,significantly longer than that of control group [（ 32. 86 ± 6. 34） min ],with statistically significant differences（ P〈0. 05）. CONCLUSIONS： Application of rocuronium bromide in laparoscopic cholecystectomy in senile patients can effectively stabilize the hemodynamic indicators with faster onset,longer duration.
