摘要
目的 分析同伴教育对伴有抑郁障碍的老年2型糖尿病患者的影响.方法 通过抑郁自评量表筛查后,选取2015年6月—2016年5月在苏州大学附属第一医院老年医学科就诊的伴有抑郁障碍的老年2型糖尿病患者60例,随机分入常规组和同伴组,各30例;同时招募血糖控制较佳的老年2型糖尿病患者30例,经培训后为同伴组患者提供一对一同伴支持.经3个月干预后,比较两组的代谢指标、积极程度、自我管理水平、情绪的变化.结果 3个月教育后,同伴组和常规组在糖化血红蛋白[(7.72±1.63)%vs(8.38±1.34)%]、积极度[(65.74±8.67)vs(51.16±9.51)]、自我管理能力[(93.6±11.7)vs(84.7±11.4)]、抑郁状况[(52.12±8.79)vs(59.76±8.46)]等方面的变化,差异具有统计学意义(P〈0.05);而在血压、血脂、体重指数等方面的变化,差异无统计学意义(P〉0.05).结论 同伴教育对改善老年2型糖尿病患者的血糖、抑郁情绪、积极度和自我管理能力具有积极意义.
Objective To explore the impact of peer education on elderly patients with type 2 diabetes and depressive disorder. Methods Sixty elderly patients with type 2 diabetes and depressive disorder were recruited for this study from June 2015 to May 2016 in the Geriatric Medicine Department of First Affiliated Hospital of Soochow University. They were divided into usual group and peer group and each group had 30 subjects. Thirty elderly type 2 diabetes patients with well glycemic control were enrolled as peer education providers, who were trained to offer one-to-one support to peer group. Metabolism index, patient activity, self-management ability and mood were compared after the three months intervention. Results Significant difference (P 〈 0.05) was found between the two groups in glycosylated hemoglobin [(7.72 ±1.63) % vs (8.38 ± 1.34) %], patient activity level [(65.74 ±8.67) vs (51.16± 9.51)], self-management skills [(93.6±11.7) vs (84.7 ± 11.4)], and depression scale [(52.12 ± 8.79) vs (59.76 ± 8.46)]; while there were no statistical significance in blood pressure, lipids and body mass index (P 〉 0.05). Conclusion Peer education can ameliorate glycemic index, depressive mood, patient activity, and self-management in elderly patients with type 2 diabetes.
