急性髓系白血病免疫表型特征与预后相关性分析

Correlative Analysis of Immunophenotypic Characteristics and Prognosis in Acute Myeloid Leukemia

目的探究伴CD56,CD7表达的急性髓系白血病(acute myeloid leukemia,AML)患者免疫表型特征。方法收集2007年1月~2016年12月来漳州市医院就诊的525例AML患者骨髓,应用四色流式细胞术,以CD45/SSC设门,检测525例AML患者白血病细胞的免疫表型。CD56^+AML和CD7^+AML作为实验组,CD56^-AML和CD7^-AML作为对照组,两组间样本率的比较用χ~2检验。结果 CD56^+AML组CD11b,CD19和CD4表达率高于CD56^-AML组,分别为31.72%vs 16.05%,22.76%vs 4.74%和35.86%vs 4.74%,差异均具有统计学意义(χ~2=14.93,36.84,85.32,P<0.01)。CD7^+AML组CD34,HLA-DR和CD117表达率高于CD7^-AML组,分别为83.20%vs 57.86%,74.81%vs51.27%和96.18%vs 89.34%,差异均具有统计学意义(χ~2=26.37,21.29,4.84,P<0.01or P<0.05);CD7^+AML组CD38,CD64,CD15和CD19表达率低于CD7^-AML组,分别为68.70%vs 82.99%,19.85%vs 41.11%,9.92%vs25.13%和2.30%vs 12.18%,差异均具有统计学意义(χ~2=11.43,18.43,12.65,9.87,P<0.01)。结论白血病的免疫表型特征对于白血病诊断、判断预后和临床治疗有重要的意义。

Objective To investigate the immunophenotypic characteristics of patients with CD56 or CD7 expression in AML.Methods 525 AML patients came to Zhangzhou Hospital from January 2007 to December 2016 were chosen.Four-color flow cytometry and CD45/SSC gating were used to detect the immunophenotypic of leukemia cells in 525 patients with AML.The experimental group and the control group were CD56 and CD7 positive or negative expression in AML respectively.The comparison of the count data between the two groups was performed inχ2 test.Results The expression of CD11 b,CD19 and CD4 in CD56 positive AML group was higher than that in CD56 negative AML group,which were 31.72% vs16.05%,22.76% vs 4.74% and 35.86% vs 4.74%,respectively（χ2=14.93,36.84,85.32,all P〈0.01）.The expression of CD34,HLA-DR and CD117 in the CD7 positive AML group was higher than that in the CD7 negative AML group,which were 83.20% vs 57.86,74.81% vs 51.27% and 96.18% vs 89.34%,respectively（χ2=26.37,21.29,4.84,all P〈0.01 or P〈0.05）.CD38,CD64,CD15 and CD19 expression levels in CD7 positive AML group were lower than those in CD7 negative AML group,which were 68.70% vs 82.99%,19.85% vs 41.11%,9.92% vs 25.13% and 2.30% vs 12.18%,respectively（χ2=11.43,18.43,12.65,9.87,all P〈0.01）.Conclusion The immunophenotypic characteristics have guiding significance in differential diagnosis of leukemia types,prognosis disease and clinical treatment.
