腹腔镜下阴式广泛子宫切除联合盆腔淋巴结清扫术治疗早期宫颈癌患者的效果分析

Analysis of the efficacy of laparoscopic radical vaginal hysterectomy combined with pelvic lymph node dissection in the treatment of patients with early cervical cancer

目的分析腹腔镜下阴式广泛子宫切除联合盆腔淋巴结清扫术治疗早期宫颈癌患者的效果。方法选取2014年1月至2017年3月郸城县人民医院收治的70例早期宫颈癌患者,按随机数表法分为两组,各35例。对照组接受开腹广泛子宫切除+盆腔淋巴结清扫术,观察组接受腹腔镜下阴式广泛子宫切除+盆腔淋巴结清扫术。对比两组术中及术后情况(术中出血量、手术耗时、拔除引流管时间、住院时间、术后排气时间)和并发症发生情况。结果观察组住院时间、引流管拔除时间、手术耗时、术后排气时间短于对照组,术中出血量少于对照组,差异有统计学意义(P <0. 05)。观察组并发症发生率(42. 85%)低于对照组(11. 42%),差异有统计学意义(P <0. 05)。结论腹腔镜下阴式广泛子宫切除联合盆腔淋巴结清扫术治疗早期宫颈癌患者,可减少术中出血量,缩短引流管拔除时间、手术耗时、术后排气时间和住院时间,降低并发症发生率。

Objective To analyze the effect of laparoscopic radical vaginal hysterectomy combined with pelvic lymph node dissection in the treatment of patients with early cervical cancer. Methods Seventy patients with early cervical cancer in Dancheng People＇s Hospital from January of 2014 to March of 2017 were randomly divided into two groups, 35 cases in each group. The control group was given the abdominal radical hysterectomy combined with pelvic lymph node dissection, and the observation group was given the laparoscopic radical vaginal hysterectomy combined with pelvic lymph node dissection. The conditions （intraoperative bleeding volume, operation time, extraction of drainage time, time of hospitalization, postoperative exhaust time） during and after the surgery- and complications of two groups were compared. Results Compared with the control group, the time of hospitalization, the extraction time of the drainage tube, the time of operation, the time of postoperative exhaust was shorter, and the amount of bleeding were lower in the observation group（P 〈 0. 05）. The incidence of complications in the observation group （ 11.42% ） was lower than that in the control group （42. 85 % ） （ P 〈 0. 05 ）. Conclusion Laparoscopic radical va- ginal hysterectomy combined with pelvic lymph node dissection in the treatment of patients with early cervical cancer can reduce intraoperative bleeding, shorten the extraction time of drainage tube, the operation time, the postoperative exhaust time and the hospitalization time, and reduce the incidence of complications.
