社区居民脑卒中患者健康管理模式的探讨

Exploring on Health Management Model of Stroke Patients in Community

目的探讨在社区中开展脑卒中患者健康管理的效果。方法选择五角场镇社区120例脑卒中出院患者,将其随机平均分为对照组和干预组。对照组患者开展自我管理,干预组患者接受规范化健康管理,为期18个月。运用日常生活能力评分标准、神经功能缺损程度评分标准、健康促进生活方式量表和健康问卷两组患者的管理效果进行评价。结果干预后,干预组患者的各项评分均比对照组高,各项指标组间差异显著(P<0.05)。结论在社区脑卒中患者中开展规范化健康管理,能够促进患者的自我健康管理水平,恢复患者肢体功能,具有较高的推广价值。

Objective To investigate the effect of health management on patients with stroke in community. Methods A total of 120 cases of stroke patients in Wujiaochang community were randomly divided into control group and intervention group. The cases in control group were carried out health self-management, and the patients in the intervention group received standardized health management conducted by community health care center for 18 months. The health management effect of the two groups was evaluated by daily life ability standard, neural function deficit score criteria, the health-promoting lifestyle profile, and health questionnaire. Results After the intervention, the scores of the intervention group were higher than those of the control group, and there was significant differences between the groups（P0.05）. Conclusion Carrying out standardized health management among stroke patients in community can promote the level of health self-management, and restore the limb function of patients, and is a high value of promotion.
