Conceptual Framework and Questionnaire Design of Healthy Behaviors Survey

目的构建上海市民健康行为调查问卷设计的概念框架,定义概念框架的维度、子维度及测量条目。方法参考WHO及国内外有关健康行为的研究,并以健康生活方式量表、中国居民健康素养监测调查问卷等为基础,通过文献回顾、专家会议和小组讨论的方法完成问卷设计和评估。结果建立了健康行为调查问卷的概念框架,包括基本健康行为、预警行为、卫生服务利用行为和避开环境危害行为4个维度,以及基本卫生习惯、饮食习惯、运动和睡眠习惯、心理健康行为等14项子维度。发展出测量条目56项,并最终形成61道问题。结论健康行为调查概念框架和问卷设计可作为测量成人健康行为的参考依据,但在应用时要注意调查对象的代表性,从而保证测量结果的真实性和有效性。

Objective To construct a conceptual framework for the design of Shanghai residents＇ healthy behaviors questionnaire, and to define the dimensions, sub-dimensions and measurement items of the conceptual framework. Methods Referring to WHO and related researches on healthy behaviors both at home and abroad, and basing on the Healthy Lifestyle Scale and the Chinese Residents＇ Health Literacy Monitoring Questionnaire, the questionnaire design and evaluation had been completed by means of literature review, expert meeting, and group discussion. Results The conceptual framework of healthy behavior questionnaire had been constructed, including 4 dimensions with basic healthy behaviors, warning behaviors, healthy service utilization behaviors, and avoiding environmental hazards behaviors, and 14 sub-dimensions, for instance, basic hygiene habits, diet, exercise and sleep habits, and psychological health behaviors etc.. Eventually, 56 measurement items and 61 question were developed. Conclusion The conceptual framework and questionnaire design of healthy behaviors survey can be used as a reference for measuring adult＇s healthy behaviors. But when applied, the representative of the respondents should be pay attention, so as to ensure the authenticity and validity of the measurement results.
