西药联合平喘固本汤治疗慢性阻塞性肺疾病急性加重期40例临床观察

Application of Western Medicine Combined with Pingchuan Guben Decoction in Acute Exacerbation of Chronic Obstructive Pulmonary Disease

目的:观察土家药加味平喘固本汤对慢性阻塞性肺疾病急性加重期患者炎症指标和氧化应激指标的影响。方法:选择80例慢性阻塞性肺疾病急性加重期患者,随机分为对照组和实验组。对照组采用常规对症治疗,实验组在对照组用药基础上加服土家药加味平喘固本汤,共治疗14 d。对比两组患者治疗前后的炎症指标(白介素-6、白介素-8)和氧化应激指标(血清丙二醛MDA、血清超氧化物歧化酶SOD)的变化。结果:治疗前两组白介素-6、白介素-8、MDA、SOD水平无差异(P>0.05),治疗后两组炎症指标水平显著低于治疗前,实验组改善效果更显著(P<0.05);治疗后实验组血清MDA水平低于对照组,血清SOD水平高于对照组,差异均有统计学意义(P<0.05)。结论:土家药加味平喘固本汤与西药联用具有协同效应,可能与调节氧化/抗氧化系统、抑制炎症反应有关。

Objective To observe the effect of Tujia medicine modified Pingchuan Gu Ben decoction on inflammatory indexes and oxidative stress indexes in patients with acute exacerbation of chronic obstructive pulmonary disease. Methods Eighty patients with a- cute aggravating period of COPD were randomly divided into control group and experimental group, with 40 patients in each group. The control group was treated with routine symptomatic treatment, while the experiental group was treated with routine symptomatic treat- ment and Tujia medicine modified Pingchuan Gu Ben Decoction. 14 days of total treatment. Contras the changes of inflammatory mark- ers （interleukin - 6, interleukin - 8） and oxidative stress （serum malondialdehyde MDA and serum superoxide dismutase SOD） be- tween the two groups before and after the treatment. Results There was no difference in the levels of interleukin - 6, interleukin - 8, MDA and SOD in the two groups before treatment （P 〉 0.05）. After treatment, the levels of inflammatory markers in the two groups were significantly lower than those before treatment, and the improvement effect in the experimental group was more significant （ P 〈 0. 05）. After treatment, the level of serum MDA in the experimental group was lower than that of the control group （P 〈 0. 05 ）. The serum SOD level was higher than that in the control group （P 〈 0. 05）, and the difference was statistically significant. Conclusion Tu- jia medicine modified Pingchuan Gu Ben decoction can regulate oxidation/antioxidant system and inhibit inflammation, and combined with western medicine has synergistic effect.
