回收式自体输血与异体输血对机体血流动力学和外周血CRP、PCT和IL-8水平的影响

Effects of Recycling Autotransfusion and Allogeneic Blood Transfusion on Serum CRP,PCT and IL-8 Levels

目的探究回收式自体输血与异体输血对机体血流动力学和外周血CRP、PCT和IL-8水平的影响。方法选取自2016年5月1日至2017年6月1日间来我院接收输血患者60例作为研究对象,采用单盲、随机设计试验,将所有研究对象按性别、手术类型、年龄等条件配对,并分为A(异体输血)、B(回收式自体输血)两组。观察两组患者围手术期失血量和输血量、不同时刻生命体征变化情况和两组患者治疗前后凝血情况相关炎症因子。结果 A组患者输血量0.85±0.12L显著高于B组0.60±0.21L,差异具有统计学意义(t=5.958,P<0.05);A组患者在T3、T4时刻MAP、HR均显著高于B组,SpO2显著低于B组,且于T2～T5时间段RR显著低于B组,差异具有统计学意义(P<0.05);A组患者术后48h PT、APTT、血清D-二聚体、PCT和血清CRP、IL-8均显著高于B组,差异具有统计学意义(P<0.05)。结论回收式自体输血能够稳定生命体征,减少术中出血,降低炎症反应,值得推广。

Objective To investigate the effects of recovering autologous blood transfusion and allogeneic blood transfusion on CRP, PCT and IL-8 levels in peripheral blood. Methods From June 1,2016 to June 1,2017,60 patients with blood transfusion were selected as the subjects. A single blind, randomized design trial was conducted to compare all subjects, sex, type of operation, age similar to the conditions for the pair, and divided into A（ allogeneic blood transfusion）, B （recovery of autologous blood transfusion）two groups. The changes of blood pressure and blood transfusion, the changes of vital signs at different times and the related inflammatory factors were observed before and after treatment in both groups. Results In group A, the volume of blood transfusion was 0.85 ±0.12L,which was significantly higher than that in group B（0.60 ±0.21L） （t =5. 958 ,P 〈0.05）. In group A,48 h PT,APTT,serum D were lower than those in group B（P 〈0.05）, and the difference was statistically significant（ P 〈 0.05 ）. Dimer, PCT and serum CRP and IL-8 in group A were significantly higher than those in group B （ P 〈 0. 05 ）. Conclusion In summary, the recovery of autologous blood transfusion can stabilize vital signs, reduce intraoperative bleeding, reduce the inflammatory response, and is worth promoting.
