猪苓汤治疗心衰终末期顽固性水肿的临床疗效分析

Clinical Analysis of Curative Effect of Zhuling Decoction on End-stage Intractable Edema of Heart Failure

目的探究猪苓汤治疗心衰终末期顽固性水肿的临床疗效。方法方便选取2014年1月—2017年6月30例心衰终末期患者,均出现顽固性水肿,均给予猪苓汤治疗。观察患者治疗前后的小腿周径、脑钠肽BNP水平、体重水平、心衰指标等实验数据。结果经治疗入组患者的小腿周径显著缩小,脑钠肽BNP水平以及体重水平均显著下降(P<0.05)。治疗后患者心衰指标检测水平均出现明显的改变,NT-proBNP、外周血管阻力指数水平显著下降,左室射血分数及心脏指数、搏出指数均显著提高(P<0.05)。治疗前后分别行心脏彩超检查,并结合患者的临床表现进行疗效判定,可得1例无效,29例好转,好转率为96.7%。入组患者均未出现治疗相关不良反应。结论猪苓汤治疗心衰终末期顽固性水肿疗效确切。

Objective To investigate the clinical efficacy of Zhuxuan Decoction in the treatment of persistent edema of endstage heart failure. Methods From January 2014 to June 2017, 30 patients with end-stage heart failure were convenient selected and treated with refractory edema, both of which were treated with Zhuling Decoction. Observed the patient before and after treatment of the calf circumference, brain natriuretic peptide BNP levels, body weight levels, heart failure indicators and other experimental data. Results After treatment, the diameter of the calf was significantly reduced, brain natriuretic peptide（BNP） level and body weight were significantly decreased（P〈0.05）. After treatment, patients with heart failure indicators showed significant changes, NT-pro BNP, peripheral vascular resistance index levels decreased significantly, left ventricular ejection fraction and cardiac index, stroke index were significantly increased（P0.05）. Before and after treatment,echocardiography was performed and combined with the clinical performance of the patient to determine the efficacy. 1 case was ineffective, 29 cases improved, and the improvement rate was 96.7%. None of the enrolled patients had any treatmentrelated adverse events. Conclusion The curative effect of Zhuling Decoction on the end-stage refractory edema of heart failure is definite.
