老年慢性心力衰竭患者健康结局与其照顾者疾病管理行为的相关性研究

Study on relationships among outcomes of elderly patients with chronic heart failure and their caregivers' disease management behaviors

目的:分析老年慢性心力衰竭(CHF)患者健康结局与其照顾者疾病管理行为的相关性。方法:选取300例老年CHF患者及其300例照料者作为观察对象,随访1年,采用CHF PRO量表评估患者的健康结局,采用家庭照顾者疾病管理行为问卷对患者的家庭照顾者进行调查,分析家庭照顾者疾病管理行为与老年CHF患者健康结局之间的相关性。结果:患者PRO总分为(160.49±27.33)分,家庭照顾者疾病管理行为问卷总分为(56.30±7.35)分,患者年龄、性别、婚姻状况、文化程度、经济状况均是CHF患者健康结局的影响因素。Pearson相关分析显示,照顾者疾病管理行为与患者健康结局呈高度正相关(r=0.568,P<0.01),回归分析显示,照顾者疾病管理行为对患者健康结局具有较强的预测作用(β=0.611,P<0.01),照顾者疾病管理行为可单独解释患者健康结局30.3%的变异量。结论:家庭照顾者疾病管理行为是老年CHF患者健康结局的预测因素。

Objective： To explore relationships among outcomes of elderly patients with chronic heart failure（CHF） and their caregivers＇ disease management behaviors. Methods： 300 elderly patients with CHF and 300 caregivers were selected as the observation subjects. Follow-up was 1 year. CHF PRO was used to assess the outcomes of the patients,and the family caregiver disease management behavior questionnaire was used to investigate the patients＇ family caregivers. Then,the relationships among the outcomes of elderly patients with CHF and their caregivers＇ disease management behaviors were analyzed. Results： The PRO total score of the patients was（160.49±27.33）,and the family caregiver disease management behavior questionnaire score was（56. 30 ± 7. 35）. The patients ＇age,gender,marital status,educational level,and economic status were the influence factors of the outcomes of elderly patients. The Pearson correlation analysis showed that the caregiver disease management behaviors were positively correlated with the patients＇ outcomes（r = 0.568,P〈0.01）. Further,the Regression analysis showed that the caregiver disease management behavior had a strong predictive effect on the patients＇ outcomes（β = 0. 611,P〈0. 01）. The caregiver disease management behavior alone can explain 30. 3%variation in the patients＇ health outcomes. Conclusions： The family caregiver disease management behaviors are the predictive factors of the outcomes of the elderly patients with CHF.
