磁共振胆胰管造影联合增强CT对良恶性胆道狭窄的鉴别价值分析

The Value of Magnetic Resonance Cholangiopancreatography Combined with Enhanced CT in The Differential Diagnosis of Benign and Malignant Biliary Strictures

目的:研究并探讨磁共振胆胰管造影联合增强CT对良恶性胆道狭窄的鉴别诊断价值。方法:选取2015年1月~2017年10月期间我院接收且明确为胆道狭窄的80例患者,对其进行磁共振胆胰管造影检查、动态增强CT检查,以手术病理诊断结果为参照,分析磁共振胆胰管造影、动态增强CT对良恶性胆道狭窄的诊断结果,并分析二者与手术病理诊断结果之间的一致性。根据手术病理诊断结果,将患者分为良性组、恶性组,比较组间CT差值。结果:磁共振胆胰管造影、动态增强CT对胆道狭窄性质的诊断灵敏度、特异度、准确性比较无统计学意义(P>0.05),且二者诊断结果与手术病理诊断结果之间的一致性均较良好。在动脉期、门静脉期以及延迟期,恶性组的CT差值均高于良性组,P<0.05。结论:在胆道狭窄的定性诊断中,综合应用磁共振胆胰管造影、动态增强CT,具有显著的鉴别诊断价值,可对良性病变与恶性病变予以准确区分。

Objective： to study and explore the value of CT in the differential diagnosis of benign and malignant biliary stricture. Methods： from January 2015 to October 2015 for our receiving and clear for 80 patients of bile duct stricture,magnetic resonance（NMR） to bile pancreatic duct imaging examination,dynamic enhanced CT examination,the surgery pathological diagnosis as reference,and analysis of magnetic resonance（NMR） bile pancreatic duct imaging,dynamic enhanced CT diagnosis of benign and malignant biliary stricture,and analyze the two consistency with surgical pathology diagnosis results. According to the pathologic diagnosis of the operation,the patients were divided into benign groups and malignant groups,and the CT differences between the groups were compared. Results： bile pancreatic duct mri imaging,dynamic enhanced CT in the diagnosis of biliary stricture properties sensitivity,accuracy,specific degrees is no statistical significance（P〉0. 05）,and the diagnosis and surgical pathology diagnosis the consistency between the both is good. In the arterial period,portal vein stage and delay period,the CT difference of malignant group was higher than that of benign group,P〈0. 05. Conclusion： in the qualitative diagnosis of biliary stricture,integrated application of magnetic resonance（NMR） bile pancreatic duct imaging,dynamic enhanced CT,the differential diagnosis of significant value,can be accurate to distinguish benign lesions and malignant lesions.
