磁共振联合血清CA125对卵巢囊腺瘤和囊腺癌的诊断价值

Diagnostic Value of Magnetic Resonance Imaging Combined with Serum CA125 for Ovarian Cystadenoma and Cystadenocarcinoma

目的:探讨磁共振扫描联合血清CA125对卵巢囊腺瘤和囊腺癌的诊断价值。方法:回顾性针对我院2015年1月至2018年1月收治并经手术病理确诊的45例卵巢囊腺瘤与28例囊腺癌患者的磁共振表现及血清CA125的关系。结果:45例经手术病理确认的卵巢囊腺瘤中,29例表现为多囊,16例表现为单囊,所有囊腺瘤囊壁及间隔表现为厚度均匀或厚度不均匀,囊腺瘤中40例CA125在正常范围,5例CA125增高,DWI序列b值800大部分病例信号不高;28例例经手术病理确认的卵巢囊腺癌中,22例表现为多囊,6例表现为单囊,均表现为囊实性肿物,其中实性部分最大径平均约4.97±3.16CM,囊腺癌中24例CA125高于正常值,4例CA125在正常范围,CA125平均值约451.84±654.56,DWI系列b值800大部分病例呈明显高信号。结论:卵巢囊腺瘤和囊腺癌在磁共振上具有特征性的表现方式,结合血清CA125,能在术前做出较为准确的定性诊断。

Objective： To explore the diagnostic value of magnetic resonance imaging combined with serum CA125 in ovarian cystadenoma and cystadenocarcinoma. Methods： The relationship between the MRI performance of 45 cases of ovarian cystadenoma and 28 cases of cystadenocarcinoma in our hospital from January 2015 to January 2018 was reviewed. The relationship between the CA125 and the serum levels of the patients with ovarian cystadenoma was studied. Results： Of 45 cases of ovarian cystadenoma confirmed by operation and pathology,29 cases were polycystic and 16 cases showed single capsule. The cystic wall and septum of all cystadenoma showed uniform thickness or uneven thickness. 40 cases of cystadenoma were CA125 in normal range,5 cases of CA125 increased,and the b value of DWI sequence was not high in 800 cases; 28 cases were confirmed by operation pathology. Of the 22 cases of ovarian cystadenocarcinoma,22 cases were polycystic and 6 cases were single sacs. All of them were cystic mass. The maximum diameter of the solid part was about 4. 97 ＋3. 16 CM. 24 cases of CA125 in cystadenocarcinoma were higher than normal,4 cases of CA125 were in the normal range,the average of CA125 was 451. 84 ＋ 654. 56,and the b value of DWI series was significantly higher in most cases. Signal.Conclusion： Ovarian cystadenoma and cystadenocarcinoma are characterized by magnetic resonance imaging. Combined with serum CA125,we can make more accurate qualitative diagnosis before operation.
