术前新辅助化疗联合手术切除治疗局部晚期食管癌的疗效观察

Curative effect observation of preoperative neoadjuvant chemotherapy combined with surgical resection for locally advanced esophageal cancer

目的探讨术前紫杉醇+顺铂新辅助化疗联合手术切除治疗局部晚期食管癌的临床效果。方法选取2014年1月至2016年12月,枣庄矿业集团枣庄医院收治的局部晚期食管癌患者78例,按治疗方式进行分组,观察组40例在手术前用紫杉醇+顺铂新辅助化疗方案治疗,再行手术治疗;对照组38例仅在术后给予同观察组的化疗方案。比较两组患者食管癌根治切除率、1年复发率。结果对照组患者根治性切除16例(42.10%),观察组患者根治性切除29例(72.50%),差异有统计学意义(P<0.05)。术后1年观察组复发6例(15.00%);对照组复发15例(39.47%)。比较两组1年复发率,观察组明显低于对照组,差异有统计学意义(P<0.05)。结论术前接受紫杉醇+顺铂新辅助化疗联合手术治疗局部晚期食管癌,明显提高食管癌根治切除率,减少姑息切除风险,降低术后1年复发率,临床疗效较好。

Objective： To study the effect of preoperative neoadjuvant chemotherapy with paclitaxel plus cisplatin regimen combined with surgical resection in the treatment of locally advanced esophageal cancer. Methods： From Jan 2014 to Dec 2016 in our hospital in the treatment of locally advanced esophageal cancer in 60 cases,were randomly divided into two groups,30 patients in the control group were given after chemotherapy treatment,the observation group of 30 cases before surgery using neoadjuvant chemotherapy with paclitaxel plus cisplatin regimen treatment,and surgical treatment esophageal cancer,the complete resection rate and the 1 year recurrence rate of two groups were compared. Results： The complete resection rate of esophageal cancer in the observation group was significantly higher than that in the control group,the 1 year recurrence rate was lower than that of the control group,the differences were statistically significant （P 〈 0. 05）.Conclusion： The clinical application of preoperative neoadjuvant chemotherapy with paclitaxel plus cisplatin regimen combined with surgical resection in the treatment of locally advanced esophageal cancer is ideal.
