肿瘤标志物CEA、CA242及CA724检测对老年肺癌病情及预后评估的临床价值分析

Clinical value of tumor markers CEA,CA242 and CA724 in the evaluation of the pathogenetic condition and prognosis in lung cancer

目的探讨肿瘤标志物癌胚抗原(CEA)、糖类抗原242(CA242)及糖类抗原724(CA724)检测对老年肺癌病情及预后评估的临床价值。方法选择我院2012年8月至2014年2月收治的老年肺癌病人70例以及同期健康体检者60例为研究对象,老年肺癌病人设为肺癌组,健康体检者设为对照组,随访时间为3年。比较肿瘤标志物CEA、CA242及CA724在2组间、在肺癌不同TNM分期间的水平差异,比较阳性病人与阴性病人中1年生存率、3年生存率的差异。结果肺癌组CEA、CA242、CA724水平均显著高于对照组,差异有统计学意义(P<0.05)。肺癌病人CEA、CA242及CA724水平随着临床分期升高而显著上升,Ⅰ~Ⅱ期肺癌病人CEA、CA242及CA724水平均显著低于Ⅲ~Ⅳ期(P<0.05)。CEA、CA242及CA724阳性病人3年生存率分别为64.58%、66.67%、66.67%,1年生存率分别为70.83%、71.93%、68.63%,均显著低于相应指标阴性病人,差异均有统计学意义(P<0.05)。结论肿瘤标志物CEA、CA242及CA724检测有助于评估病人病情程度和预后水平,值得推广。但是由于本研究样本量小、随访时间短,因此深入研究诊断价值有待于进一步验证。

Objective To investigate the clinical value of tumor markers carcinoembryonic antigen（ CEA）,carbohydrate antigen（ CA） 242 and CA724 in the evaluation of the pathogenetic condition and prognosis of lung cancer. Methods A total of 70 elderly patients with lung cancer admitted from August 2012 to February 2014 in our hospital and 60 healthy subjects were enrolled in this study. All subjects were followed up for 3 years. The levels of CEA,CA242 and CA724 were compared between two groups and between different TNM stages of lung cancer. 1-year survival rate and 3-year survival rate were recorded and compared. Results The levels of CEA,CA242 and CA724 in lung cancer group were significantly higher than those in control group（ P〈0. 05）. The levels of CEA,CA242 and CA724 in the patients with stage Ⅰ-Ⅱ lung cancer were significantly lower than those in the patients with stageⅢ-Ⅳ（ P〈0. 05）. The 3-year survival rates and 1-year survival rates of positive patients of CEA,CA242 and CA724 were significantly lower than those of negative patients respectively（ P〈0. 05）.Conclusions The detection of tumor markers CEA,CA242 and CA724 is helpful to evaluate the pathogenetic condition and prognosis of the elderly patients with lung cancer.
