摘要
目的:探究经直肠腔内超声三维成像术在术后肛周感染治疗效果中的评估价值。方法:选取诊断为术后肛周感染的59例患者进行直肠腔内超声三维成像检查,并与手术结果进行对比分析。结果:59例患者中手术证实为单纯性肛周脓肿43例、肛瘘6例、复合型肛周脓肿10例,三维超声检查单纯性肛周脓肿41例、肛瘘9例、复合型肛周脓肿9例,与手术结果一致性高(Kappa=0.628);三维超声检查单纯性肛周脓肿及肛瘘的敏感度、特异度、准确度对比差异均有统计学意义(P<0.05),使用超声诊断单纯性肛周脓肿、肛瘘及复合型肛周脓肿中超声回声特点具有差异。结论:使用直肠腔内超声三维成像术对术后肛周感染具有较高的准确率和特异性,能够较好地区分肛周脓肿类型。
Objective: To investigate the value of transrectal three - dimensional ultrasound imaging in the treatment of perianal infection after surgery. Methods:59 patients diagnosed as perianal infection were enrolled in this study. Three- dimensional (3D) imaging of intrarectal ultra- sound was perforated and compared with the surgical results. Results: Among the 59 patients, there were 43 cases of simple perianal abscess, 6 ca- ses of anal fistula, and 10 cases of complex perianal abscess confirmed by operation. There were 41 cases of simple perianal abscess, 9 cases of a- nal fistula, and 9 cases of complex perianal abscess by three - dimensional ultrasonography. The results of three - dimensional ultrasonography showed that the sensitivity, specificity and accuracy of three - dimensional ultrasonography for simple perianal abscess and anal fistula were statisti- cally significant ( P 〈 0.05 ). The difference of ultrasonic echo characteristics using ultrasound in the diagnosis of simple perianal abscess, anal fistula and compound perianal abscess was significant. Conclusion : The three - dimensional rectal ultrasound imaging of perianal infection with postoperative infection has a higher rate and specificity, which can distinguish between the types of perianal abscess, with high clinical value.
