Three Hospitals in Previous Five Years: a Retrospective Analysis

目的分析某军医大学3所附属医院医疗纠纷特点规律,结合军民融合发展趋势,提出预防处理医疗纠纷的对策。方法对某军医大学三所附属医院2012—2016年医疗纠纷进行回顾分析,并与地方同等级医疗机构进行对比。结果军医大学附属医院医疗纠纷发生呈下降趋势,纠纷主要发生在外科、骨科、内科和急诊科等重点科室,妇产科、口腔科、眼科、耳鼻咽喉科等专科纠纷发生风险较高。以非医疗性原因为主,主要采取医患协商途径解决,非理性维权发生率较低。结论军医大学附属医院应完善军民融合下医疗纠纷处理机制,做好重点科室和高风险专科纠纷预防,积极开展医务人员人文素养和沟通技能培训,加强医疗质量管理、医疗风险预防和早期介入。

Objective To analyze the characteristics and rules of medical disputes in affiliated hospitals of a military medical university and propose suggestions to prevent medical disputes under civil military integration development. Methods A retrospective analysis of medical disputes in affiliated hospitals in previous five years was made and differences were compared between military hospitals and civil hospitals. Results The incidence of affiliated hospitals was declining,medical disputes generally incidence occurred in departments such as surgery,orthopedics,internal medicine and emergency while gynaecology and obstetrics,stomatology,ophthalmology and E. N.T department had higher risk of medical disputes. Non-medical factors were the main reason,negotiation between physician and patients was the main solution,and incidence rate of irrational rights protection was low in affiliated hospitals of the military medical university. Conclusion The main preventive measures are perfecting civil and military integration medical disputes solving system,enhancing prevention in key departments and high-risk departments,improving humanistic quality and communication skills training among medical staff,and strengthening medical quality management,medical risk early warning,and early intervention.
