大气颗粒物与肥胖关系的系统综述

摘要 目的采用定性研究方法对PM_(2.5)和PM_(10)污染与肥胖发病率相关性研究进行系统综述。方法使用Cochrane Library、Pub Med、万方数据库、维普中文科技期刊数据库、中国知网等数据库,就2017年7月5日前发表的有头PM_(2.5)和PM_(10)浓度变化与肥胖发病率关系文献进行检索。以NOS量表和AHRQ评价标准评估纳入文献的偏倚风险,分别提取PM_(2.5)和PM_(10)导致肥胖发病的比值比(odds ratio,OR)、危险比(hazard ratio,HR)及其95%CI和相关数据。结果共纳入文献5篇,与PM_(2.5)有关的文献3篇,与PM_(10)有关的文献2篇;其中3篇为人群队列研究,2篇为横断面研究。3篇与PM_(2.5)有关的文献均认为,PM_(2.5)对肥胖的发病率或患病率及体重增加无明显影响;2篇与PM_(10)有关的文献中,1篇认为女性肥胖患病率的增加与PM_(10)浓度每增加IQR(19μg/m^3)相关,其OR值为1.18(95%CI:1.06~1.32),另1篇文献认为PM_(10)对肥胖患病率无影响。文献质量评价为低偏倚风险。结论根据目前人群流行病学研究结果,PM_(2.5)或PM_(10)暴露与肥胖无明显关联。 Objective To understand the relationship between ambient particulate matter and obesity. Methods The literature searching on relationship mentioned above was performed by using Cochrane Library, Pub Med, Wanfang, VIP and CNKI Database before July 5, 2017. The eligible studies were identified and assessed for quality according to the NOS scale and AHRQ evaluation criteria. The odds ratio(OR), hazard ratio(HR), and so on were obtained. Results A total of five articles were accepted, assessed as low bias risk.Three articles were related to PM(2.5), which all considered that effects of PM(2.5) on the incidence or prevalence of obesity and weight gain were not significant. Two articles were related to PM(2.5), one considered that the increased prevalence of obesity was associated with an interquartile range increase in PM(10)(19 μg/m^3; OR=1.18, 95%CI:1.06-1.32), while another one considered not. Conclusion No significant association can be seen between obesity and PM(2.5) or PM(10) based on current epidemiological studies.
