Practical Application of DRG in Performance Evaluation of Hospital Departments

摘要 目的:探讨DRG在医院科室绩效考核中的实践应用。方法:以BJ-DRGS (2014版)为分组方式,建立能力、效率和医疗安全3个考核维度。结果:样本科室覆盖18个MDC分组,95个DRG分组,占样本医院DRG总组数的12.2%;收治病例的难度(CMI为0.727)处于全院的较低水平;消耗指数、费用消耗指数均接近1,处于全院的平均水平;医疗安全方面,1人死亡,未存在低风险组死亡病例。结果与结论:DRG在医院科室绩效考核中的应用,从产能、效率和安全3个方面进行考核,改变了科室管理仅关注科室工作量指标和经济效益指标情况,利于引导临床科室加速周转、降低成本,控制医疗支付费,提升服务能力和效率同时强化医疗质量。 Objective: To explore the practice of Diagnosis Related Group(DRG) in the performance evaluation of hospital departments. Methods: 3 assessment dimensions of ability, efficiency and medical security were established according to BJ-DRGS(2014). Result: Selected departments covered 18 MDC groups, 95 DRG group, accounting for 12.2% of the total number of DRG in target hospital. The diffieuhy of the hospitalized patients(CMI=0.727) was at a low level of the hospital, consumption index and cost index were close to 1, which was at the average level of the whole hospital. As for medical safety, there was 1 death and no low-risk ease of death. Results and Conclusion: The application of DRG in the performance assessment of hospital department was evaluated from the aspects of productivity, efficiency and safety, which brought changes in the management of departments only focus on the workload indicators and economic performance indicators, was helpful to guide clinical departments to speed up turnover and reduce costs, control medical payment fees, improve service ability and efficiency and strengthen medical quality.
