胃安汤联合三联疗法治疗消化性溃疡(肝胃不和)随机平行对照研究

Randomized Controlled Parallel Study of Weian Decoction(胃安汤) combined with Triple Therapy in the Treatment of Peptic Ulcer(Ganwei Buhe/肝胃不和)

[目的]观察胃安汤联合三联疗法治疗消化性溃疡(肝胃不和)疗效。[方法]使用随机平行对照方法,将72例门诊患者按就诊顺序号方法随机分为两组。对照组38例奥美拉唑,20mg/次,2次/d,2周后改为1次/d;阿莫西林,1g/次,2次/d;枸橼酸铋钾,240mg/次,4次/d,三餐前30min服用,第四次睡觉前服用。治疗组38例胃安汤(丹参30g,白术、白芍各20g,党参、山药各15g,柴胡、茯苓、枳壳、白及、乌贼骨、郁金各10g,陈皮、元胡、木香~煨各6g,甘草、黄连各3g),水煎300mL,1剂/d,早晚温服1次;西药治疗同对照组。连续治疗8周为1疗程。观测临床症状、胃黏膜溃疡、不良反应。治疗1疗程(4周),判定疗效。随访1年,观测复发率。[结果]治疗组临床痊愈22例,有效12例,无效4例,总有效率89.74%;对照组临床痊愈10例,有效12例,无效16例,总有效率57.89%;治疗组疗效优于对照组(P<0.05)。不良反应治疗组低于对照组(P<0.01)。随访1年,复发率两组无显著差异(P>0.05)。[结论]胃安汤联合三联疗法治疗消化性溃疡(肝胃不和),疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of Weian decoction combined with triple therapy on peptic ulcer （Ganwei Buhe）. [Methods] Using random parallel control method, 72 outpatients were randomly divided into two groups according to the order number method. In the control group, 38 cases of omeprazole, 20 mg/times, 2 times/d, 2 weeks later to 1 time/d; amoxicillin, 1 g/times, 2 times/d; bismuth citrate, 240 mg/times, 4 time/d, before three meals 30rain, fourth times before sleep. In the treatment group, 38 cases of Danshen 30g （Danshen 30g, Baizhu, Baishao each 20g, Dangshen, Shanyao each 15g, Chaihu, Fuling, Zhiqiao, Baiji, Wuzeigu, Yujin each 10g, Chenpi, Yuanhu, Weimuxiang each 6g, Gancao, Huanglian each 3g）, decoction 300mL, 1 time/d, 1 times in warm clothes, and 1 times in the morning and evening; western medicine was treated with the control group. Continuous treatment for 8 weeks was the 1 course of treatment.Clinical symptoms, gastric mucosal ulcers and adverse reactions were observed. The 1 course of treatment （4 weeks） was treated and the curative effect was determined. The recurrence rate was observed for 1 years. [Results] In the treatment group, 22 cases were cured, 12 cases were effective, 4 cases were invalid, the total effective rate was 89.74%, 10 cases were cured, 12 cases were effective, 16 cases were invalid, the total effective rate was 57.89%, the curative effect of the treatment group was better than the control group （P 〈0.05）. The adverse reaction treatment group was lower than the control group （P〈0.01）. Follow up for 1 years showed no significant difference in recurrence rate between the two groups （P〉0.05）. [Conclusion] The curative effect of Weian decoction combined with triple therapy on peptic ulcer （Ganwei Buhe） is satisfactory, and there is no serious adverse reaction. It is worth promoting of popularization.
