一清胶囊联合米诺环素治疗慢性牙周炎的临床研究

Clinical study on Yiqing Capsules combined with minocycline in treatment of chronic periodontitis

目的探讨一清胶囊联合盐酸米诺环素软膏治疗慢性牙周炎的临床疗效。方法选取2016年5月—2017年10月武警河南省总队医院收治的慢性牙周炎患者96例为研究对象,按照随机数字表法分为对照组和治疗组,每组各48例。对照组在牙周袋内注入盐酸米诺环素软膏,使药物填满牙周袋包裹牙齿,1次/周。治疗组在对照组基础口服一清胶囊,1.0 g/次,3次/d。两组均连续治疗3个月。观察两组的临床疗效,比较两组的牙周指标和血清炎症因子水平。结果治疗后,对照组和治疗组的总有效率分别为70.83%、87.50%,两组比较差异具有统计学意义(P<0.05)。治疗后,两组龈沟出血指数(SBI)、附着丧失(AL)、探诊深度(PD)、牙龈指数(GI)、菌斑指数(PLI)均显著降低,同组治疗前后比较差异具有统计学意义(P<0.05);且治疗组牙周指标明显低于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组C反应蛋白(CRP)、白细胞介素1β(IL-1β)水平均显著降低,同组治疗前后比较差异有统计学意义(P<0.05);且治疗组血清炎症因子水平明显低于对照组,两组比较差异具有统计学意义(P<0.05)。结论一清胶囊联合盐酸米诺环素软膏治疗慢性牙周炎具有较好的临床疗效,能改善牙周指标,降低血清IL-1β、CRP水平,具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Yiqing Capsules combined with Minocycline Hydrochloride Ointment in treatment of chronic periodontitis. Methods Patients（96 cases） with chronic periodontitis in Armed Police Corps Hospital of Henan from May 2016 to October 2017 were randomly divided into control and treatment groups, and each group had 48 cases. Patients in the control group were injection administered with Minocycline Hydrochloride Ointment in the periodontal pocket, and filled the periodontal pocket with the drug, once weekly. Patients in the treatment group were po administered with Yiqing Capsules on the basis of the control group, 1.0 g/time, three times daily. Patients in two groups were treated for 3 months. After treatment, the clinical efficacies were evaluated, and periodontal indexes and serum levels of inflammatory factors in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 70.83% and 87.50%, respectively, and there was difference between two groups（P〈 0.05）. After treatment, SBI, AL, PD, GI, and PLI in two groups were significantly decreased, and the difference was statistically significant in the same group（P〈 0.05）. And the periodontal indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups（P〈 0.05）. After treatment, the levels of CRP and IL-1β in two groups were significantly decreased, and the difference was statistically significant in the same group（P〈 0.05）. And the serum levels of inflammatory factors in the treatment group were significantly lower than those in the control group, with significant difference between two groups（P〈 0.05）. Conclusion Yiqing Capsules combined with Minocycline Hydrochloride Ointment has clinical curative effect in treatment of chronic periodontitis, can improve periodontal indexes, decrease the levels of CRP and IL-1 β, which has a certain clinical application value.
