摘要
目的探索利用废弃埋藏式心脏转复除颤仪器(ICD)进行心房高频刺激制作犬心房颤动(简称房颤)模型的可行性。方法 6只杂种犬在全麻下经右侧颈内静脉途径植入右心耳和/或右室电极,项背部皮下植入废弃ICD,经皮下隧道将起搏电极与ICD脉冲发生器连接。1周后开始通过体外程控仪器进行50 Hz高频刺激诱发房颤。记录每次刺激后诱发房颤的能力和房颤发作持续的时间。阵发性房颤定义为房颤发作持续时间2~30 min。持续性房颤定义为房颤持续时间超过30 min。结果 6只犬均成功植入起搏系统,5只犬完成房颤模型制作。采用高频刺激制作阵发性房颤模型需要累积刺激18(12,25)次,发生在第3.6(3,5)日;而制成持续性房颤模型需要累积刺激97(83,117)次,所需时间18.6(15,20)日。结论利用废弃ICD行心房高频刺激可成功制作出犬房颤模型,具有方法简单、经济实用等优势。
Objective To investigate the feasibility of atrial fibrillation(AF) canine model that is made by the high frequency stimulation with abandoned implantable cardioverter defibrillator(ICD). Methods Six canines were im planted with pacing electrode in right atrium or/and right ventricular through right internal jugular vein under gener- al anesthesia. The ICD generator was buried under the back of neck, and connected with the pacing electrode by the subcutaneous tunnel. High frequency stimulation with 50Hz was remotely released with programmable instrument. AF events were recorded in ICD and calculated after every stimulation to evaluate the inducing capability of high fre- quency stimulation. Paroxysmal AF was defined as AF lasting for 2--30 minutes, and persistent AF was defined as AF lasting for more than 30 minutes. Results All 6 canines were successfully implanted with pacemaker system, and 5 of 6 were successfully made with AF model. Paroxysmal AF model was archived with mean 18(12, 25) stim- ulations on mean 3.6(3, 5) days. Persistent AF model was archived with mean 97(83, 117) stimulations on mean 18.6(15, 20) days. Conclusion Canine AF model can be established successfully by high frequency stimulation with abandoned implantable cardioverter defibrillator, which have the advantages of simple method and practical e conomy.
