基于医院HIS系统真实世界的血必净注射液临床应用研究

The Clinical Research into Clinical Application of Xuebijing Injection

目的:基于医院HIS系统探讨血必净注射液真实世界的临床应用情况,为规范合理用药提供参考。方法:收集医院信息系统(hospitai information system,HIS)近3年来使用过血必净注射液的住院患者信息,运用统计学方法对使用科室分布、患者一般信息、诊断信息、临床用药信息等进行分析。结果:血必净临床使用集中在急诊科、呼吸重症科和普外科;用药患者多为50岁以上中老年人,占78.86%;手术患者占65.74%,非手术患者占34.26%;危重患者占32.26%;单次用药剂量多为50 m L或100 m L,占93.26%;用药疗程3~12 d,占50.46%;临床多与头孢菌素、蛋白酶抑制剂及益气固脱类中成药注射剂等联用。结论:血必净注射液临床使用较多且适应证较广,多用于中老年、术后和危重患者。临床存在联合用药较多,超适应证使用情况,可能存在用药风险,应根据临床实际使用情况做好血必净注射液的风险管理工作。

Objective： To probe into the clinical application of Xuebijing Injection（ XBJI） in order to provide reference for standardizing rational drug use. Methods： The information of the impatients who had used XBJI in the past 3 years was collected through the hospital information system（ HIS）. The was collected nearly 3 A to use the inpatient information of XBJI. Statistical methods were used to analyze the distribution of the department which used XBJI,the general information of the patients,the information of the diagnosis,and the information of the clinical medication. Results： The clinical application of XBJI focused on in emergency department,respiratory intensive care unit and department of general surgery. Most of the patients were over 50 years old,accounting for 78. 86%; the surgical patients accounted for 65. 74%,the non-surgical patients accounted for 34. 26%,the critically ill patients accounted for32. 26%. The single dose was 50 m L or 100 m L,accounting for 93. 26%,and the course of treatment was often between 3 and 12 days,accounting for 50. 46%. It was often used in combination with cephalosporins,protease inhibitors and Chinese patent medicine injections for nourishing qi and preventing exhaustion. Conclusion： XBJI is widely used in middle-aged,elderly patients and critically ill patients. In clinical practice,it is often combined with other drugs,and there is the use of super indications,which may lead to drug-use risks. Therefore,the risk management of XBJI should be done according to the actual situation of clinical practice.
