慢性肾小球肾炎脾肾阳虚证研究述评

Review of Chronic Glomerulonephritis with Yang Deficiency Syndrome of Spleen and Kidney

慢性疾病患者,正气虚损尤甚,即所谓"久病必虚",导致病人形成动力缺失的一系列症状,脾失固摄,肺失宣降,肾不固精导致精微下泄,水液运行乏力致湿浊痰瘀,久而化热化毒。故在治疗上,重视固护阳气,切忌轻易泻火损伤阳气,制方遣药上应注重温补元阳,即使是邪实为重者,也不可多用寒凉、渗泄药物,以犯"虚虚"之过,须知邪实乃是正虚的结果。而今人们生活节奏加快,熬夜、饮食不规律、过食辛辣油腻,致脾胃阳气损伤,临床治疗更应该注意,且莫再以寒凉之药物攻伐中焦阳气。在临床上不乏有医家一见患者有面红,口干,脉大等"热象",不假斟酌,就已处方数味寒凉伤阳之品,殊不知有真寒假热证,故今后应注重固护真阳和扶助正气方面的研究,则慢性肾炎的中医治疗必将有更广阔的前景。

Yang qi plays a vital role in the physical function of the whole body,is the motive force of the life activity,the maintenance to keep the balance between yin and yang. For the chronic diseases of the human body,during the process of the long-term dispute between healthy qi and pathogenic qi,healthy qi is especially damaged,that is,the so-called＂long-standing illness leading to deficiency＂,which causes the patients to form a series of symptoms of the lack of power： The spleen fails to consolidate and control. The lung fails to ascend and descend qi. The kidney fails to store essence leading nutrients to purge down.Water retention,dampness,turbidity and phlegm are stagnated,and at last heat and toxicity are formed. Therefore,the treatment,should pay attention to protecting yang qi,avoiding purging fire damaging yang qi. The prescription should focus on warming and nourishing kidney yang,even if pathogenic factor is serious,cool and cold herbs should also be avoided,because excessive pathogen results from deficient healthy qi. Today,people＇s life rhythm is accelerated,staying up late even all night,eating irregularly,eating spicy and greasy are common,causing spleen and stomach yang qi injured,but the clinical treatment should avoid cool and cold attack. In clinical,there are no lack of doctors who immediately give patients with reddish face,dry mouth,and large pulse the medicine which is cool and cold in nature,they really don＇t know there is cold syndrome with pseudo-heat symptoms. Therefore,in the future,we should pay attention to protecting true yang and strengthening the direction of healthy qi in the research,the treatment of chronic nephritis will have a broader prospect.
