介入技术在产科应用32例临床分析

Involvement in technology applied in obstetric 32 cases of clinical analysis

目的分析对于产科出血病患,使用介入性技术治疗的临床价值。方法择取2015年5月~2017年5月本院收治的32例产科疾病患者,其中包含子宫颈妊娠6例,剖宫产术后瘢痕部位妊娠16例,产后出血10例。结合患者病情,对宫颈妊娠患者,栓塞治疗后,肌肉注射甲氨蝶呤+口服米非司酮;对产后出血患者开展双侧子宫动脉栓塞术治疗。分析治疗效果以及不良反应发生情况。结果所有患者均择取右侧髂内动脉开展数字减影血管造影治疗。出血诊断标准为造影剂外溢位置。本组患者经子宫动脉栓塞治疗,手术均获得成功。患者痊愈出院。开展相关治疗后,3例病患术后发生不良反应,完成治疗后短时间内好转。治疗有效率为100.00%。结论对于产后出血患者,开展超选择性子宫动脉栓塞术+子宫动脉灌注药物+栓塞术治疗,能够全面显示出出血血管部位,准确进行栓塞,使用超选择性方式对该处血流进行阻塞处理,以取得预期治疗效果,由此能够看出,对于产后出血性疾病者,使用介入性治疗,临床效果显著,安全性强,不良反应少,值得进一步在临床中推广使用。

Objective To analyze the clinical value of interventional therapy for patients with obstetric hemorrhage. Methods From May 2015 to May 2017, 32 patients with obstetric diseases were selected, including 6 cases of cervical pregnancy, 16 cases of postoperative scar pregnaJlcy af- ter cesarean section, and 10 cases of postpartum hemorrhage. Combined with the patient＇s condition, for the cervical pregnancy patients, after emboli- zation treatment, intramuscular injection of methotrexate ＋ oral mifepristone; For patients with postpartum hemorrhage, bilateral uterine artery embo- lization was performed. To analyze the treatment effect and the occurrence of adverse reactions. ReSults All patients were selected to use the right internal iliac artery for digital subtraction aaagiography. The diagnostic criteria for hemorrhage was the location of contrast extravasation. All the patients in this group were successfully treated by uterine artery embolization. The patient was cured and discharged. After the relevant treatment, 3 patients experienced postoperative adverse reactions, which were improved within a short time after the completion of the treatment. The effective rate of treatment was 100.00%. Conclusion For patients with postpartum hemorrhage, to carry out the super selective uterine artery embolization ＋ ＋ embolization, uterine artery infusion drugs can fully show the bleeding blood vessels, accurate to embolism, using super selective way to block the blood flow to the registry, to achieve the therapeutic effect, thus can see, for the postpartum hemorrhagic disease, use of interventional treatment, clinical effect is remarkable, strong safety, less adverse reactions, worthy of further promotion in clinical use.
