高龄患者行逆行胰胆管造影术治疗的围术期安全性护理

Research on the Safety Nursing of ERCP in Elderly Patients

目的探讨高龄胆胰疾病患者行逆行胰胆管造影术(endoscopic retrograde cholangiopancreatography,ERCP)治疗的围术期安全性护理。方法回顾性分析2016年1月至2017年12月因胆胰疾病接受ERCP治疗的172例高龄患者(80～97岁)临床资料,分析其操作成功率、并发症率及病死率,总结ERCP围术期安全护理要点。结果ERCP操作成功169例(98.3%);发生并发症11例(6.4%),其中发生胰腺炎7例、胆管炎3例、出血1例,无穿孔等其他并发症发生;无死亡病例。结论高龄胆胰疾病患者行ERCP治疗是安全可行的,围术期安全护理需重视术前评估,掌握术中配合要点,术后及时发现并处理并发症,这是保证治疗成功的重要条件。

Objective To analyze the safety and perioperative nursing care of endoscopic retrogradecholan giopancreatography （ERCP） in the treatment of pancreaticobiliary disease in elderly patients. Methods The clinical data of 172 elderly patients （80 to 97 years old} with biliopancreatic diseases treated with ER CP from January 2016 to December 2017 were analyzed retrospectively. The procedure outcomes,complication and mortality were analyzed,and the key points of perioperative safety nursing of ERCP were summa rized. Results There were 169 （98. 3%） successful ERCP procedures and 11 （6. 4%） patients developed complications, including 7 cases of pancreatitis,3 cases of cholangitis,1 case of hemorrhage. There were no perforation and other complications,and no procedure related death. Conclusions ERCP is safe and feasible for elderly patients with pancreaticobiliary disease. Perioperative safety nursing should pay attention to preoperative evaluation,the key points of intraoperative cooperation and proper treatment of postoperative complications,which are the important conditions to ensure the success of treatment.
