四维彩超、二维彩超在胎儿畸形诊断中的临床应用价值

Clinical application value of four-dimensional color Doppler ultrasound and two-dimensional color Doppler ultrasound in the diagnosis of fetal malformation

目的比较四维彩超、二维彩超在胎儿畸形诊断中的临床应用价值。方法选取我院产科收治的110例畸形胎儿孕妇作为研究对象,对孕妇分别进行四维彩超检查和二维彩超检查,观察胎儿畸形情况。结果四维彩超检出畸形胎儿108例,诊断准确率为98.18%;二维彩超检测出畸形胎儿84例,诊断准确率为76.36%,两种检查方法的诊断准确率、误诊率及漏诊率比较,差异显著(P<0.05)。四维超声对胎儿心脏、四肢、躯干畸形的检出率及总检出率均显著高于二维超声,差异具有统计学意义(P<0.05)。结论四维彩超对胎儿畸形的检出率高于二维彩超,所形成的四维立体活动图像更易辨认,值得推广。

Objective To compare the clinical value of four-dimensional color Doppler ultrasound and two-dimensional color Doppler ultrasound in the diagnosis of fetal malformation. Methods One hundred and ten cases of pregnant women with malformed fetuses admitted in obstetrics department of our hospital were selected. Four-dimensional color Doppler ultrasonography and two-dimensional color Doppler ultrasonography were performed to observe the fetal malformation.Results A total of 108 cases of malformed fetus were detected by four-dimensional color Doppler ultrasound, with the diagnostic accuracy rate of 98.18%, 84 cases of fetal malformation were detected by two-dimensional color Doppler ultrasound, with the diagnostic accuracy rate of 76.36%. The diagnostic accuracy rates, misdiagnosis rates and missed diagnosis rates of the two examination methods were significantly different（P〈0.05）. The detection rates of fetal heart, limbs and trunk malformation and total detection rate of four-dimensional color Doppler ultrasound were significantly higher than those of the two-dimensional color Doppler ultrasound（P〈0.05）. Conclusion The detection rate of fetal malformation by four-dimensional color Doppler ultrasound is higher than that of the two-dimensional color Doppler ultrasound, and the four-dimensional stereoscopic motion image formed is more recognizable, which is worthy of promotion.
